This page shows the selection of Products
available on Direct Proposal Journey. To
go through this page, select ‘Direct’ after
selecting Prospect on My Clients page.

• Health Care Access is ONLY available


through Direct Proposal Journey in
Native APP.

Note: User cannot continue the journey on


the assessment created on web.

• Click Health Care Access to proceed to


the journey
Upon clicking Health Care Access, there
will be a prompt message reminding users
that the product is ONLY exclusive for
clients living in the Philippines.

There will be two options available.


1. Choose another product – this will let
the user select another product on
the Direct Proposal Landing Page

1. Proceed to Proposal – this will


redirect to About You page of Health
Care Access proposal journey.

About You Page
About You page will capture basic details
of Insured and/or Policy Owner. the Policy
Owner can input work details on this page
if he/she select Yes on the question "Are
you the person to be insured?"

The Policy Owner has also an option to add


a different Proposed insured.
Select “No” to add a different Proposed insured
On the Occupation field, there is a validation
for a declined occupation.
An error pop-up message will prompt.

Select an acceptable occupation and input


details on all fields.

About Your
Dependents Page
Adding dependent/s
• This page will capture basic details about
the dependents if any.
• User selects Yes to add dependent/s
• User selects No if client do not want to add

a dependent on the plan.
User has an option to add dependent/s.
• Click "Yes" on the question "Would you like to
add members to your family plan?“ to see the
form for dependent/s
• User can add maximum of 10 dependents
• Dependents must be 0-60 y.o
• A single insured cannot add sibling and child at
the same time
• A single insured can only add Siblings who are
single, unemployed and less then 18 y.o
• A single insured can add a child below 18y.o

• Click " to proceed to the Take over or


declaration of existing health page

(Takeover Policy)
This page will capture details about insured/dependents existing health coverage.

• To declare an existing health coverage, user should select “Yes” on the question
"Do you and/ or family have an existing health cover?

• When "Yes" is selected, user can select insured person with Existing
Health coverage to be insured.
• User can select “Yes” to question "Do you want to switch your
existing healthcover with AXA?"

• If "Yes" is selected, user can inpunt the details of the existing health
coverage

• Only the plan of insured and dependents with existing health


coverage are considered as Takeover
• And only those considered in the Takeover will proceed to the plan
creation
• Click "Next" to proceed to the selection of HCA Plan type

1. Health Care Access Prime – a
plan which has Inpatient Care,
Outpatient Care, Longevity
Health Fund and Dental Care

2. Health Care Access Lite- a plan
with Inpatient Care and
Longevity Health Fund

They can also select preferred


payment mode; Quarterly, Semi-
annual, Annual. This page also
displays, computed premiums and
discount (if any).

1o% Discount is computed for a


family plan only

NOTE:
NO selection of annual deductible for
HCA Prime as this by default non-
deductible type of plan

Can only input agent code that is the same

On this page the user can review the summary of the proposal. It shows the Health Plan Benefits, Benefit coverage and Premium
Breakdown.

• User clicks the button to Sync

Select "Yes" to redirect to the


Sync Manager page

user is redirected to the Sync Manager page

Click to upload the date and


proposal.
Uploaded proposal from Native will create an
assessment on web but is not viewable.

Successful Sync message will be displayed

After successful Sync,

Proposal in PDF form.

Proposal will be displayed.


Proposal contains the all the details inputted
and created on the proposal journey.

Pressing the Submit Application will redirect the


application to the Eapplication journey

Personal Information

• Once syncing of Proposal is completed, there


will be an option in Assessment Log to Submit
Application. Once click, it will redirect to the
Eapplication – Personal Info page.

• Capture basic info from proposal journey will


be auto-populated/displayed on the left panel
of this page and will not be editable (also
displayed in Mailing Information and Source
of funds screen)

• On the right panel, this will capture additional


details of insured and/or Policy owner and
dependents(if any)

• Accepted Country of Residence is only


Philippines. Government ID and ID Number is
only required for Policy Owner/Principal
Insured and Dependents who are older than
18 yrs old

• Press Next to save and proceed to Mailing

Mailing Information

This page will capture mailing details of insured


and/or Policy owner and dependents(if any).
Residence/Present address, Permanent Address,
and Business address(except juvenile dependent
or principal insured) is required for (Policy owner,
Principal Insured, and dependents).
Province, city, and country is auto populated
upon entering zip code(drop down is available
once a few digits of the zip code is entered)

Preferred Mailing Address is only available for


Policy Owner and Principal Insured

Press Next to save and proceed to Source of


Funds

Source of Funds

This screen will capture details regarding the


monthly income of policy owner and principal
insured(if PO<>PI) including the other sources of
income if it is applicable to them

Details regarding the recurring payment option is


also captured(Payment type, payment mode, and
bank). Chosen payment mode and payment type
in recurring payment cannot be changed once
the eapp is synced

Regular Premium reflected is based on the


payment mode(regular premium will also change
when payment mode is changed)

Press Next to save and proceed to Underwriting

This screen will capture the answers of the


insured and dependents regarding other
insurance details.

Insured/dependents that did not answer will be


tagged as NO answer in EAPP Form(this is
applicable to all UW Questions and Client Details
questions)

Insured/Dependents can only be chosen


once(will be removed from the list when a new
person to be insured is added for the question)
for each question(this is applicable to all UW
Questions and Client Details questions)

secondary beneficiary(can be distributed
between multiple beneficiary) but it is not
required

Can add beneficiary using the select from


dependent or by manually adding beneficiary

This screen will capture the E-Signature


of the user. Policy Owner,Principal
Insured, and adult dependents have the
option to provide their e-signature

If e-signature is provided, Signed in the


Philippines tickbox needs to be checked

adult dependents also have the choice
to skip e-signature

Press Skip e-signature to skip providing


e-signature and proceed to the eapp
sync

Press Next then press Yes when asked


"Do you want to save signature?" To
save the signature and to proceed to

and the result of the sync if it is successful or

If all mandatory requirements are uploaded press


Submit Application to proceed to submit
application sync(pop up will appear, press yes to
proceed)

This screen lets the user

If the user saves their e-signature prior to


the eapp sync, e-signature will already be

payment (cash and credit card can be
chosen) and Recurring
payment(selected in Source of funds
during the eapp journey)

When cash is selected for initial and


user proceeds with the application, will
be redirected to Initial Payment where
user can select between the offline
payment proof to upload

When credit card is selected for initial


or recurring payment and user
proceeds with the application, will be
redirected to peso pay. After the
transaction, will be redirected to the
upload requirements main screen

Click view application to view the eapp

While viewing the app, tap the upper


right elipsis to see the save to device and
save to email button

Press save to device to save the eapp


form(PDF) in the device used

Press save to email to receive the eapp


form(PDF) via email

Image on the lower right part of the slide


is a sample of a downloaded/saved eapp
in PDF form
