Re: Approval of Student Assistantship

We are pleased to approve your Student Assistantship application under UTARRPS funding. You
are required to serve the Project Vote No. 6251/T12, “Development of the self-rated artistics
creativity scale” under the supervision of Dr. Tan Chee Seng at the Faculty of Arts and Social
Science (FAS), Kampar Campus, Universiti Tunku Abdul Rahman.

Payment claim shall be made monthly by completing the Student Assistantship Claim form.
Other terms and conditions are as stated in the Guidelines on Student Assistantship.

This student assistantship shall be terminated at the expiry of this student assistantship or any time
on occurrence of any of the following:
(i) Your services are no longer required in the project or the job assigned to you under the
project has been completed.
(ii) The work for which you are engaged to perform the services has been suspended or
ceased.
(iii) Any gross misconduct, insubordination and persistent absence from work without
approval on your part.

Please indicate your acceptance of this appointment by signing and returning the Form UTAR-
IPSR-SA-G002 to the undersigned immediately, failing which this offer of appointment is withdrawn
and void.
