I, MAXIMA O. CRUZ of legal age, Filipino, and with resident address at 18G. ROTC St.
Rosario, Pasig City, do hereby NAME, CONSTITUTE AND APPOINT, daughter PERLITA OCAMPO
CRUZ of legal age, Filipino, and with residing at 8G. ROTC St. Rosario, Pasig, to be my true and
lawful ATTORNEY-IN-FACT, for me and in my name, place, and stead to do and perform the
following act by my/our representative/Attorney-In-Fact, to wit:

1. To claim/secure/get my check from LANDBANK and to encash the said check with
Check No. 0000750926 and the amount of ₱ 3,041.90 at Land Bank East Ave. Branch
SSS Livelihood Trade CENTER Bldg. East Ave. Diliman Q.C;
2. To open a joint account ATM with my daughter in landbank;
3. To sign/deliver any/all forms/documents/papers that maybe required pertinent to
such aforementioned powers;
4. To do and perform any/all other acts/deeds as maybe required/necessary to affect
performance of such powers granted to my said ATTY-IN-FACT.

HEREBY GIVING AND GRANTING unto my said ATTY-IN-FACT full powers and authority to
do and perform any all acts that maybe required or necessary to carry into effect the foregoing
authority as fully to all intents and purposes as if I was personally present and acting in person
and HEREBY RATIFYING AND CONFIRMING all that my said ATTY-IN-FACT shall do or cause to
be done by virtue hereof.
