I, MUSTAFA NAS , of legal age, Turkish national and a resident of

Zamboanga City, after having been duly sworn in accordance with law, hereby
depose and say:

1. That I was employed as Mathematics Teacher for Elementary Level

in Tolerance School from July 14, 2014 to May 15, 2019, as
evidenced by a Certificate of Employment, hereto attached as Annex
“A”;

2. That said Certificate of Employment is requested for authentication.

3. That I hereby attest to the authenticity and genuineness of the said

document ;

4. I am executing this affidavit to attest to the truth of the foregoing

allegations and for the purpose stated above.
