Tomas, Pasig City, hereby appoint and name EMILY SALBEDIA GALIBO, of legal age, Filipino and
residing at Purok #5 Market Ave, San Miguel, Pasig City,to be my true and lawful attorney-in-
Fact, for me and in my name, place and stead, to do and perform the following acts and things
to wit:To exercise full control with my document regarding on my Retirement such as
clearances and other;

1. To transact to any concerned agencies regarding on the said Retirement such as,
GSIS, and City Hall of Pasig City, and other agencies concerned;
2. To follow-up and inquire Statement of Accounts, Statement of Loan Accounts and
other documents related thereto;
3. To sign and execute on my behalf any and all documents and formalities which the
Attorney-in-fact considers to be necessary to or conducive to the execution of any of
the powers set out in this general power of Attorney;
4. To settle/make payments of any and all charges fees related to the said Retirement;
5. To receive cash or checks with the GSIS and City hall and to encash or withdraw the
given amount under my name;
6. To sign/execute and deliver any/all forms/documents/papers that maybe required
pertinent to such aforedescribed powers;
7. To do and perform any/all other acts/deeds as maybe required/necessary to effect

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority
to do and perform all and every act, requisite or necessary to carry into effect the foregoing
authority, as fully to all intents and purposes as I might or could lawfully do if personally
present, with full power of substitution, and hereby ratifying and confirming all that my said
attorney-in-fact shall lawfully do or cause to be done by virtue hereof.
