AFFIDAVIT OF BEING SINGLE PARENT

I, EDROLYN L. MANALO, of legal age, single parent, and a resident of 247

Dr. Pilapil St., San Miguel, Pasig City, after having duly sworn in accordance with
law, hereby depose and say:

1. That I am the mother of MARK MIJO MIGUEL MANALO, born out of

wedlock on January 7, 2017 at Rizal Medical Center;

2. That I have been solely responding to the financial or needs and daily
existence of my said son;

3. That at present I have no job and only taking care of my little boy;

4. That I am executing this affidavit to attest to the truth of the foregoing

allegations and for whatever purpose it may serve.
