AFFIDAVIT AS CO-OWNER AND A VETERINARIAN

I, DR. EMMA LUISA ESQUERRA, of legal age, single, Filipino and a resident
of No. 27 Camia St., Twin Ville subd. Concepcion I, Marikina City, after having
been duly sworn under oath, hereby depose and say:

1. That I am a Veterinarian by profession, with PRC Lic. No. 5897;

2. That I practice my profession as Veterinarian at the Pasig Line Animal

Clinic located 21 CPL Bldg. Mercedes Ave., San Miguel, Pasig City, and as
co-owner of said Animal clinic;

3 . That I am executing this affidavit to attest to the truth of the foregoing

statements and for whatever purpose it may serve.
