AFFIDAVIT

I, RIGOR J. ENRIQUEZ, of legal age, Filipino, and a resident of Barangay San

Nicolas, Pasig City, after having been duly sworn under oath, hereby depose
and say:

1. That I am the Barangay Captain of San Nicolas, Pasig City;

2. That sometime on December 14, 2009, our Barangay under the City
Government of Pasig City, Acknowledged Receipt for Equipment one
(1) unit of Utility Vehicle, a Mitstsubishi ; with Plate No. SKL801, 2010
model; and with CR. No. 11754722-0 & CR No. 993097335;

3. That said vehicle appears to have a blue color when we received the
same.

4. That after a thorough inspection of said vehicle, it was found out that
the original color is WHITE, which we are not aware of it;

5. That I am executing this affidavit to attest to the truth of the foregoing

allegations and for whatever purpose it may serve.
