AFFIDAVIT OF ACKNOWLEDGMENT/ADMISSION OF PATERNITY

I, ROBBIE MARK GENERILLO OBISPO, of legal age, Filipino citizen, and with residence
and postal address at 122 Burol Main, City of Dasmariñas, Cavite, Philippines, having been
duly sworn to in accordance with law, under oath and avers:

1. That I am the biological father of BLAKE GRIFFER ASIS who was born on 10
December 2011 in the City of Manila and the facts of his birth were duly reported and
recorded at the Local Civil Registrar of the City of Manila with Registry Number
2012-001378;

2. That at the time of birth of my son, I and Honelyn A. Asis, the biological mother of
my son were not legally married;

3. That we were legally married on 21 February 2012 in the City of Dasmariñas, Cavite;

4. That as a natural father of my son, I hereby acknowledge and declare that he is my

true and legal child;

5. I hereby acknowledge my paternity to BLAKE GRIFFER ASIS and recognize them

as my child and legal heir;

6. Further, I allowed the above-named child to use my surname pursuant to R.A. 9255
and shall be known as BLAKE GRIFFER ASIS OBISPO;

7. That I hereby certify that the statements made herein are true and correct to the best
of my knowledge and belief;

8. That I have executed this Affidavit to establish my paternity over the child and for
whatever legal purposes and intents the same may serve best.
