Please accept this letter as my formal resignation from my position as Job Order Medical
Technologist at the South Cotabato Integrated Provincial Health Office. Effective September 1,
2021.

The decision to pursue another opportunity has been difficult, as being in the Public Health
Institution has been a positive experience for me. It’s been a pleasure working with you for the
past 1 year and 2 months. I have enjoyed doing the tasks entrusted to me for the betterment in
providing healthcare services to the Province of South Cotabato. I am grateful for the
opportunities to develop my skills in the field and I want to thank you for all your guidance and
insight.
