penalties/sanctions and/or shut down work, without prejudice to legal remedies and claim for damages by Declarant/Association/Estate Management.

(d). I further authorize the representatives of Estate Management to enter the site at all times, for inspection purposes.
(e). In case there is a need to relocate and/or renovate the above mentioned project because of future Estate developments, I agree to
undertake all the responsibility and cost arising from the said relocation and/or renovation.
(f). Construction Guarantee Deposit/Bond (as applicable): I hereby acknowledge that any such fines or costs charged against the
deposit/bond shall be without prejudice to whatever works or actions that may be required of me by Estate Management to rectify or remedy the
violation, breach or infraction so committed at my sole cost and expense.
(g). I have the authority to sign on behalf of and therefore to bind, the corporation/partnership/principal.

THIS PERMIT IS HEREBY ISSUED SUBJECT TO THE FOLLOWING TERMS AND CONDITIONS:

1. The work shall be done in the most expenditious manner so as to minimize
inconvenience to the public. In this regard, you shall coordinate with MaCEA at
least four (4) hours prior to the actual implementation of the project.
2. Work shall be done in such a manner as to avoid accidents. Adequate warning
and informative signs (duly approved by MaCEA) for the safety of the general
public should be installed and placed such that obstruction is minimized.
3. In the execution of your work, exteme care shall be exercised to prevent
damage on the existing facilities and/or utilities and all other properties of
MaCEA. Measures should be taken so that concrete and its residue, and other
construction debris, will not drain into the drainage system. Any damage caused
to such properties shall be repaired by you and at your own expense, in
accordance to our accepted standards.
4. Upon completeion of your work, you shall leave the area affected by this
project free of all excess or waste materials and restore the same to its original
neat condition. Excess materials shall be hauled within four (4) hours after the
completion of the work. You shall notify MaCEA within twenty-four (24) hours
after your completion of the project.
5. Noise generating activities are not allowed from 8:00 PM to 8:00 AM on
Mondays to Saturdays and whole day on Sundays and Holidays
6. Displaced parking slots shall be coordinated and paid to the Makati Parking
Authority (MAPA).
7. Personnel capable of handling vehicular traffic should be assigned on a full time
basis to avoid creating traffic problem.
8. Sewage shall not be permitted to be discharged into the drainage system.
9. All workers must properly wear their company uniform complete with personal
protective equipment or PPE. Wearing of sanos/sleeveless shirts, slippers/sandals
and other outfits that do not exhibit proper decency are not allowed outside the
construction sites.
10. MaCEA's notation on the submitted drawing should be strictly complied with.
11. You shall comply with all existing laws, government regulations and
requirements concerning this project
12. A copy of permit must be kept at the worksite at all times and it must be
presented to MaCEA representatives upon demand.
13. Subject to the inspection by MaCEA representative.
14. This permit may be cancelled or revoked anytime if any of the terms and
conditions listed herein are violated.
15. MaCEA is hereby relieved and released from any and all liabilities and
responsibilities from any clamis resulting from your undertaking.
16. Additional Terms and Conditions:
16.1 For lot, sidewalk or road portion temporary enclosure:
a.) The enclosire should be made of good quality G.I. sheets or
plywood, at least eight (8) feet high, structurally sound and
luminously painted for the safety and convenience of the general
public. No commercial sign shall be [painted on or attached to the

c.) Cleaance from MAPA should be secured prior to the usage or installation
of driveway access.
16.2 For repair/renovation/maintenance of a building affected sidewalk areas:
a.) The sidewalk should remain safe and freely passable to pedestrian at all
times.
b.) Removal/disposal of debris shall be the responsibilty of the building
owner/contractor.
16.3 For power, telecom and other utilities installations:
a.) You shall secure Right-of-Way permit from the Ayala Property
Management Corporation (APMC) prior to the implementation of the
project.
b.) Work is allowed during Mondays to Saturdays (except holidays) from
9:00 PM to 6:00 AM of the following day and 8:00 AM to 8:00 PM during
Sundays and Holidays. c.) No
service vehicle shall be parked at the work area. Manhole coves should be
rigidly fastened upon work completion.
16.4 For road excavations:
a.) You shall secure Right-of-Way permit from the Ayala Property
Management Corporation (APMC) prior to the implementation of the
project.
b.) Prior to undertaking the work, you shall remit to MaCEA the amount of
(P______________) as payment for the cost of restoring affected areas
(please refer to construction assestment at the front page)
c.) Prior to undertaking the work, you shall remit to MaCEA the amount of
(P______________) as deposit (without interest) for the cost of restoring
other areas may be damaged during the course of the work. (please refer
to bond assestment at the front page)
d.) Upon completetion of your work, excavated areas shall be backfilled at
every 0.20 meter layer with at least 95% compaction, Results of field
density tests shall be submitted.
e.) As-built plan shall be submitted one (1) week after the completion of the
project.
f.) All conditions on MaCEA's Guidelines for Road Excavations constitute an
integral part of this permit and should be complied with accordingly.
16.5 For stationing of construction vehicles, transit mixers, trucks or other type of
vehicle or equipment on roadway.
a.) The roadway shall be maintained safe/passable to all types of vehicles at
all times. Total road closure is prohibited.
b.) The vehicles and equipment should be confined within one lane of the
roadway nearest the project site, and should be stationed one vehicle-at-a-
time only, in order not to disrupt the normal flow of traffic. No
vehicle/equipment may be parked to occupy more than one lane of the
roadway. Power cables, where necessary, shall be in such a way that it is
safe and will not cause tripping/obstruction to the pedestrians.
c.) For delivery trucks, buses, vans: Engine must be turned off while
parked/waiting.
