AFFIDAVIT OF CONFORMITY

That I, JOSE D. DELA CRUZ of legal age, Filipino, married, with residence at La
Roxas, Maramag, Bukidnon, after having been sworn to in accordance with law do
hereby depose and say:

1. That the subject of this instrument is the land award by the Department of
Agrarian Reform bearing Title No. AO-702 or CLOA No. 00046471, registered
under Esteban Lomod, et., al., with an area of sixty thousand five hundred
seventeen (60, 517) square meters, located in San Roque, Maramag,
Bukidnon. The said CLOA is herein attached as Annex “A”;

2. That I am one of the annotated co-owners in the aforementioned CLOA;

3. That upon actual survey conducted by the DAR, it was found out that the
Area showing in the Title awarded by DAR is different from that of the
Approved Subdivision Plan, evidenced by the ASP No. Psd-10-042684 (AR),
which is hereto attached as ANNEX “B”. (For the CLOA, SEE ANNEX “A”);

The following discrepancies are as follows:

4. That I conform that the area showing in the Certificate of Land Ownership
Award ( CLOA ) is an estimate and that, actual tillage is confirmed through
the actual survey;

5. That the boundaries stated in the Certificate of Land Ownership Award and
the actual survey as reflected in the Approved Survey Plan are one and the
same.

This affidavit is being executed to attest to the foregoing facts and in compliance
with the requirements prescribed by the Department of Agrarian Reform.
