Please notify everyone concerned (subcontractors and suppliers) as to the issuance and
contents of this Addendum prior to the date of bid opening. This Addendum is a part of the
contract documents and modifies them as follows:
GENERAL
I. Plan Holders List: A copy of the current plan holders list is attached to this addendum.
II. Pre-Bid Conference Meeting Minutes: A copy of the Pre-Bid Conference meeting
minutes and sign-in sheet are included in this addendum.
III. Substitutions: This is an acceptance of general quality only. No attempt has been
made to check each material as to special features, capacities, or physical dimensions
specially required for the project. It shall be the responsibility of the supplier,
manufacturer, and contractor to check all requirements before submitting for final
acceptance. Final acceptance of exact features, sizes capacities, etc., all of which must
match materials indicated and specified, will be determined when submitted during the
construction period. Certain acceptances are subject to conditions noted.
GENERAL
I. REVISE the project building address to 2606 ½ (this is the address the Contractor will
need to put on their building permit application to IDOPL).

A. DRAWING A2.0
1. See attached and replace this sheet in its entirety (revisions are
revclouded).
2. See photo sketch below and refer to the attached Drawing A2.0 for
additional grading to be provided by the Contractor. The intent of this
grading is to allow the same surface drainage to occur but instead of the
water flow meandering and then ponding up against the new asphalt
driveway, the Contractor shall regrade that gravel area to sheet flow
south and over the new asphalt driveway.
B. DRAWING A3.0
1. See revised Detail 17/A3.0 below showing revised metal flashing installation.
Provide continuous 2x pressure treat nailer along sill plate as shown below.

Construction Estimate:
Bid Alternates:
• None.
A Public Works license is required of all contractors and subcontractors working on the project. A bid bond
of 5% is required at time of bid (bid bond of 5% to include base bid and all alternates). Performance and
payment bonds will be required.
Bid Proposal:
Complete ALL items on the Bid Proposal (do NOT leave any spaces blank or bids will be deemed ‘non-
responsive’). The Bid Proposal includes the “Contractor’s Affidavit Concerning Alcohol and Drug-
Free Workplace” and “Bidders Acknowledgement Statement.” These are to be completed and
included with the contractor’s bid proposal (6 pages total). Acknowledge the number of addenda
received. Do not leave spaces blank. Do NOT use white out to modify the bid form. Initial any bid changes
on the form. General Contractors are required to name their Mechanical, Plumbing and Electrical
subcontractors on the Bid Proposal as indicated.
Building Permits:
• The Contractor is responsible for and shall secure and pay for the Building Permit required by the
Division of Occupational and Professional Licenses (DOPL) – formerly the Division of Building Safety
(DBS). The Owner has paid for associated plan review fees for this project. The electrical, plumbing,
and mechanical contractors are responsible for and shall secure and pay for the subcontractor
permits.
• ALL PERMITS AND INSPECTIONS ARE STILL BEING COORDINATED THROUGH THE DBS
WEBSITE (www.dbs.idaho.gov) AND NOT THE DOPL WEBSITE (www.dopl.idaho.gov).
• Contact DBS or refer to their website to determine the estimated permit fee amount. Per DBS’s
permit application process, Contractors are responsible for calculating actual Permit Fee based on
their respective bid amounts (‘Total Building Construction Value’).
• The Building Permit application is available on DBS’s website and may be submitted to them by fax,
email, or delivered in person. For assistance registering or processing permits, contact the DBS help
members at 1-800-955-3044. Contractor needs to use site address on permit application (NOT the
Agency address).
• Maintain access to existing facilities. Do not close or obstruct facilities without written permission
from Owner and approval of authorities having jurisdiction.
• Notify Owner not less than 48 hours in advance of activities that will affect Owner's operations.
• Staging and Laydown Area: there is a large amount of available area at the work area and directly
adjacent to the work area. After bid-award, at the pre-construction meeting, the exact location of
the Contractor’s staging and laydown area will be determined and will be coordinated to meet the
Contractor’s needs for construction.
• The importance of maintaining a safe, secure and clean work area is emphasized. Clean up and
securing of the site is to be maintained and is the contractor’s responsibility on a daily basis.
• The Contractor is responsible to provide and pay for quality control testing for this project from a
certified testing agency, for the following items:
o Sub-grade inspections.
o Backfill and Compaction
o Reinforcing Steel and Placement
o Concrete Placement and Curing
o Asphalt Paving & Sub Base
