CP - Pyramidal

HPI:

From the time of birth until one year in 2015 to 2016, the mother of the pt. noticed
that the pt. still can't crawl, sit, and walk on her own, also noticed that the pt's legs
are crossed (hip education) due to tightness of muscles. The mother of the pt. was
worried about the developmental delay of her kid, so she went to the hospital for
a clinical check-up to know what is the condition of her kid. Pt. went undergone a
CT scan, (date and place unrecalled). At that time, pt. was getting regular check-
ups, but still diagnosis cannot be given.

In 2018, Pt. was diagnosed with Cerebral Palsy (Spastic Quadriplegic) by Dr. S, at
LORMA Medical Center. The mother of the pt. went to Baguio General Hospital to
get a second opinion, and got the same diagnosis. Pt. was referred to Dr. M a
physical therapist at LORMA Medical Center to treat muscle tightness. Pt. did get
any medications for therapy. Pt. continues on treatment up to the present.

In 2023, Pt. was diagnosed with B hip dislocation by Dr. T. at LORMA Medical
Center. Pt went undergoing regular check-ups for hip dislocation. the mother of pt.
went to Baguio General Hospital for second a opinion and was recommended for
surgery for the pt.'s hip dislocation. Pt. goes to physical therapy twice a week at
LORMA Medical Center (see PT Mx). Pt.
