-IV frusemide
-1.5 L fluid restriction
-Spironolactone 25mg ceased and started on Nebivolol
-1.25mg daily

Mild global systolic dysfunction; estimated LVEF 43 + 5%. Mild compression of the LV cavity due to
flattening/bowing of the interventricular septum .. Moderate tricuspid regurgitation. Severe pulmonary hypertension
with estimated RVSP 83mmHg .. Severe right ventricular dilatation with severe systolic dysfunction. Small
posterior pericardia! effusion, of no haemodynamic consequence .. The patient was transferred to the GVH
emergency department for urgent inpatient evaluation and stabilisation.

1.D/C Home
2.GP follow up in 2/52 with repeat UECs
3.Cardiology follow up (SCS) for Pulmonary Hypertension work up
