Mental Status Report 1
Patient seen and examined awake, alert and slightly cooperative. Patient look older
than her stated age. Patient is poorly groomed and noted to have strong body odor with
unkempt and matted hair. Wearing a gray shirt and maroon shorts. She’s unable to
maintain eye contact, with notable downward gazing. Patient spoke in a soft, low voice,
with regular rate and rhythm with multiple pauses in between. When the patient was
asked how she was, patient replied with “wala” with blunted affect. Formal though
disorder noted were tangential and though blocking. Patient claims to have been
hearing voices in her head but claims to be incomprehensible “dili masabtan” but denies
visual hallucinations and delusions. Patient is oriented to person but not to place and
time. Was unable to elicit response for memory, concentration, abstract-reasoning, and
judgment as the patient was unresponsive.
