In accordance with the requirements of the FSMS Standard (ISO 22000:2018 Clause 5.3.1c)
I am happy to inform you that management has appointed you as a Food Safety Team Member (FSTM)
for AK General Dealers Lumwana project effective 1st January, 2022.

Your responsibilities and authority as the FSTM will be as a follows;

 To carry out the hazard analysis with the team

 Establish, maintain and update flow diagrams as documented information for the products or
product categories and the processes covered by the FSMS.
 Confirm on-site the accuracy of the flow diagrams, update the flow diagrams where appropriate and
retain as documented information
 Validate that the selected control measures are capable of achieving the intended control of the
significant food safety hazard(s).
 Conduct an analysis of the results of verification that shall be used as an input to the performance
evaluation of the FSMS
 Where required, modify and re-assess the control measure(s) and/or combination(s) of control
measure(s).
 Maintain the validation methodology and evidence of capability of the control measure(s) to achieve the
intended control as documented information
 Evaluate the FSMS at planned intervals to consider whether it is necessary to review the hazard
analysis, the established hazard control plan and the established PRP’ s.

Kindly be advised that this is not a promotion, therefore your current position remains unchanged and you
shall perform the rest of your responsibilities as per your job description
