We, the Grade 12 Senior Highschool students of Far Eastern College-Silang (FEU Cavite) from the strand

Science, Technology, Engineering and Mathematics (STEM), are currently conducting a research entitled

“Comparison of the Effects of Physical and Digital Mediums on the Student's Note-Taking Willingness

in Math Class” in partial fulfillment of the requirements in the Research Project and Practical Research 2.

The purpose of this research is to identify which is more effective; the use of physical or digital mediums

in increasing the willingness of students to take notes during math classes.

With your expertise, we humbly ask your permission to validate the research instrument entitled

Comparison of the Effects of Physical and Digital Mediums on the Student's Note-Taking Willingness

in Math Class’s Pre-survey Questionnaire on or before March 23, 2023. The researchers will provide the

file of the instrument that will be given upon your approval or request.
