Dear SHS Parents/Guardian/s,

Greetings!

On behalf of the FEU Cavite Senior High School (SHS) Department and in lieu of the National Arts Month
2021 Celebration, I am delighted to inform you that your son/daughter is one of the selected students for
the SIPAT LENTE: QUARANTHINGS contest. This contest aims to help students to showcase their talent in
writing and photography as well as to document their ‘discovered hobbies or creative past times’ during
the quarantine period. In line with this, his/her selected photo entry with Tanaga caption will be uploaded
on the official Facebook Page of FEU Cavite.

In connection to this, if you are allowing your son/daughter’s output to be publicly posted, please
accomplish and submit the parent’s permission form on or before February 23, 2021, 5:00 PM. Kindly
send the accomplished form to amanansala@feucavite.edu.ph .

for the holistic development and learning of your son/daughter. We also hope and pray that FEU Cavite

in celebration of the National Arts Month 2021.

I also understood that the Department reserves the right to exclude my child in the online
program in my inability to submit this waiver on time.
