Subject: #YOUTH4BIODIVERSITY LEARNING HUB AND

ONLINE HACKATHON

The Office of the Undersecretary for Administration (OUA) through the

Bureau of Learner Support Services – Youth Formation Division (BLSS-YFD), in
cooperation with YGOAL Inc., invites all Youth for Environment in Schools –
Organization (YES-O) and Supreme Student Government (SSG) Officers to the
#Youth4Biodiversity Learning Hub Sessions on 22-23 April 2021 at 2:00-4:00
p.m. and #Youth4Biodiversity Online Hackathon on 28-30 April 2021 via
Facebook Live at https://www.facebook.com/ygoal .

YGOAL Inc. is a social enterprise that delivers high-impact programs on

organizational development, training and development, and impact assessment for
individuals. These activities aim to engage school-based and community-based
youth organizations to become more active agents of change to address biodiversity
loss and climate crisis.

In this regard, regional and division offices are requested to provide the
necessary support and assistance to participants for the entire duration of these
events which includes the submission of the accomplished parent/guardian
consent form (Annex A).

PARENT/GUARDIAN CONSENT FORM

This form confirms that WE agree to allow the participation of my

son/daughter in the #Youth4Biodiversity Learning Hub Sessions on 22-23
April 2021 at 2:00-4:00 p.m. and #Youth4Biodiversity Online Hackathon
on 28 April 2021. This is to confirm that WE give full permission for any
recording that may be done during this training and for use of some or
all their image/contribution/performance in any publication (including
electronic publications such as film or website) created by or for the Youth
Formation Division (YFD) and to release this material to the Department of
Education’s official platforms.

I hereby confirm that I agree and understand the commitment of my

son/daughter as an Elected Federation Officer. I also understand and will
support my son/daughter’s endeavor to meet the expectations, guidelines,
and responsibilities to his/her fellow students and to the YFD.
