Chapter 1. INTRODUCTION
Before servicing the portable bedside monitor CAPNOSTREAM™20P, please read the information in this
manual. This manual applies to the service of CAPNOSTREAM™ by authorized service engineers only.
Only authorized service engineers are permitted to service CAPNOSTREAM™ Devices.

1.1. Audience
The intended audience of this manual is authorized service personnel.

1.2. About the System


CAPNOSTREAM™20/ 20p is a portable dual-parameter bedside monitor that integrates Oridion
Microstream™ Capnography technology and pulse oximetry technology developed by Nellcor™ or
Masimo™, depending on product version. For the sake of brevity, the device will be referred to in this
document.
The monitor provides continuous, non-invasive measurement and monitoring of the carbon dioxide
concentration of expired breath (EtCO2), inspired breath (FiCO2), and respiration rate (RR). This monitor
also provides arterial oxygen saturation (SpO2) and pulse rate (PR). These measurements are available
both real time and in trend summaries. The monitor stores trend data for 72 hours. The monitor displays
data in numeric on a 6.4" color TFT screen, and also displays the CO2 and SpO2 waveforms. The monitor
has graded and color-coded visual and audio alarms.
Monitor parameters can be set in three levels:
• Values set by the operator/nurse and applicable until the monitor is switched on.
• Values by the user/biomedical engineer and applicable at startup.
• Factory presets to which the user can revert.
This allows hospitals and clinics to configure the monitor based on the environment in which it will be used.
In addition, it also allows the individual operator to override these institutional preferences. The data is
available for output via RS232, flash drive specific USB, a built-in two-channel thermal printer and a 7-
channel D/A. CAPNOSTREAM™ devices have a 2.5-hour battery backup.

1.3. System Software


The monitor software resides in Flash Memory. It can be upgraded externally through the USB port using
the Software Download facility.
The CO2 module software can be upgraded externally through the RS232 port when the monitor is
configured for CO2 Direct Communication.
Refer to the Service Screen (see Figure 54. CAPNOSTREAM™ monitor Service Screen on page 67) for
software and hardware version of the monitor and modules. (Refer to 6.1Service Support on page 67.)
The software is being periodically updated. It is recommended to always use the updated software if
applicable.

Chapter 2. SAFETY
Read the information in this manual before attempting to service CAPNOSTREAM™ devices. This chapter
explains the safety requirements with respect to servicing.

2.1. Electrical Safety


This equipment has been designed to meet IEC 60601-1 standards and the corresponding national
standards. It has Class I type of protection against electrical shocks, degree Type 'BF' as applied parts.

2.1.1. Warnings
• Use insulated tools while adjusting controls.
• Do not open the system enclosures in power ON condition.
• All internal adjustments and replacements must be done by qualified technician or service
personnel.
• Do not operate the system without the back cover in the presence of flammable gases or
anesthetic agents. This could lead to an explosion.
• Disconnect the AC power cable from the system before cleaning system.
• Inspect the AC power adapter cord and plug on a regular basis. Ensure that they are not damaged.
• Use properly grounded equipment. Shock hazards might occur if the AC power adapter is not
properly grounded. Grounding reliability can only be achieved when equipment is connected to a
receptacle marked "Hospital Only" or "Hospital Grade" or the equivalent. The grounding wire must
not be removed.
• Use accessories and peripherals recommended for CAPNOSTREAM™ monitors per instructions in
the User Guide.

2.1.2. Caution Notes


The following precautions can help reduce electro-static discharge (ESD):
• Anti-static spray on carpets.
• Anti-static spray on linoleum.
• Anti-static mats.
• Anti-static wrist bands.
• Proper grounding of the work area.
If the device has a back-panel switch, turn OFF the system by switching the power ON/OFF hot key on the
front panel and the ON/OFF switch on the rear panel. If the device does not have a back-panel switch, turn
OFF the system by switching the power ON/OFF hot key on the front panel and unplugging the device.
To avoid rise in temperature of the system, do not block the airflow to the ventilation holes on the back
panel of the system.

2.2. Equipment Safety
2.2.1. Caution Notes
• Be sure to turn OFF the power (using the back-panel switch, if your device has such a switch, and
the front panel ON/OFF button) and remove the power plug from all equipment before working on
it.
• Excessive bending or twisting of cables can cause a failure or intermittently affect the operation of
the device.
• Take care when connecting or disconnecting cables. A damaged cable can cause a short in the
electrical circuit.
• Do not plug in or remove PCBA cards while the unit is in ON condition.
• Improper cleaning or disinfecting any part of the system with non-recommended solutions can
cause permanent damage (Refer to 2.3 Cleaning and Disinfection, below.)
• Do not use solvents such as thinner or benzene or abrasive cleaners on any part of the system.
• Do not spill liquid on the system.
• Do not handle PCBAs without proper static protection. Improper handling may damage
components.
• Do not assemble or configure incorrectly. Use proper power source.
• Prevent damage to the connectors by aligning connector pins before you connect the cable.
Misaligned connector pins can cause damage to system components at power ON.

2.3. Cleaning and Disinfection


To clean the monitor's surfaces, lightly dampen a cloth with a 70% alcohol solution and wipe all surfaces.
Alcohol wipes may also be used. Frequency of the cleaning procedure should be in keeping with hospital
policy.
To clean the screen, use a damp, lint-free cloth.

2.3.1. Caution Notes


• Do not autoclave or sterilize this device.
• Do not spray or pour any liquid directly on the monitor, accessories or consumables.
• Do not use caustic or abrasive cleaners, or harsh solvents, including petroleum-based or acetone
solutions, to clean the device.
• Microstream™ EtCO2 consumables are designed for single patient use and are not to be reused.
Do not attempt to disinfect or flush the FilterLine™ as the monitor can be damaged.
• If a 1:10 bleach dilution (0.5% to 1% Sodium Hypochlorite solution) in the form of wipes is used to
disinfect external surfaces of the monitor, exposure of the connectors and the display to the bleach
solution should be avoided. Repeated cleaning with bleach over time may cause discoloration and
residue on the surfaces.

2.4. Electro-static Discharge (ESD) Protection


ESD is defined as the transfer of charge between bodies at different electrical potentials. This can change
the electrical characteristics of a semiconductor device, degrading or destroying it. ESD also may upset the
normal operation of an electronic system, causing equipment malfunction or failure.
The damage done by ESD may cause the immediate failure of the unit or result in performance
degradation and eventual failure.
The instruments can be damaged by the electric field radiating out from an electrostatic body. MOS

technology devices are most susceptible to voltage or electrostatic field damage. Bipolar and TTL devices
are more susceptible to current damage.

2.4.1. ESD Protection Materials

Chapter 3. SYSTEM ARCHITECTURE
3.1. System Description
The monitor is designed as a portable patient monitor for easy transportation and clear viewing of
waveform and parameters. The unit works on AC Mains and on internal battery. It provides RS232-based
communication with a PC and USB memory stick data transfer.
It consists of following main functional groups:
• Main board
• SpO2 Module
• CO2 Module
• Power Supply
• Display Unit
• Keyboard and Optical Encoder
• Accessories

3.1.1. Main Board


The Main board is based on MCF5270 microcontroller (32-bit), a part of the ColdFire® family of reduced
instruction set computing (RISC) microprocessors.
Its main features include:
• Provides System debug support
• On-chip Memories - 8-Kbyte cache & 64-Kbyte dual-ported SRAM on CPU internal bus
• Three Universal Asynchronous Receiver Transmitters (UARTs), two of which communicate with
the Thermal Printer and Debug port, and the third is optional.
• I2C Module which communicates with the power supply regarding battery information
• Queued Serial Peripheral Interface (QSPI)
• Four 32-bit DMA Timers
• Four Periodic Interrupt Timers (PITs)
• Software Watchdog Timer
• Frequency Modulated Phase Locked Loop (PLL)
• DMA Controller
• External Bus Interface - Glueless connections to external memory devices (e.g., SRAM, Flash,
ROM, etc.); 32-bit bi-directional data bus; 24-bit address bus
• Chip Configuration Module (CCM)
• General Purpose I/O interface
• JTAG support for system level board testing
• Fast Ethernet Controller (FEC)
The Main board also consists of:
• Flash Memory containing the boot loader and image (OS kernel + application software)
• SDRAM from which the code executes.
• Non-Volatile RAM containing data such as institutional default settings, trend data, etc. that must
be retained even after power OFF.
• Real Time Clock (part of the NVRAM) which maintains system date and time.
• Display Controller that provides interface between the microprocessor, EDORAM, and TFT display.
• EDORAM which stores data for display controller.
• USB Controller that provides the interface between Main board and USB port.
• External UART with asynchronous communication elements which communicates with the

modules and RS232 port.
• Keyboard interface which provides the interface between Main board and keyboard/optical encoder
• Analog Output section, which provides Digital to Analog converted signals.

3.1.2. CO2 Module


This module is produced by Oridion. It communicates with the Main board with a serial protocol providing a
CO2 waveform, inspired/expired carbon dioxide and respiration rate, which are refreshed on the display.
The method for measuring CO2 is based on non-dispersive IR absorption of the CO2 in the breath sample,
using the Oridion IR source.

3.1.3. SpO2 Module


This module is a product of either Nellcor™ Inc., USA, or Masimo™ Corp., USA technology, depending on
product version. It communicates with the Main board on serial protocol, providing SpO 2 waveform, PR,
SpO2%, Signal IQ Waveform (feature specific to Masimo™ monitors) and SatSecondsTM (feature specific to
Nellcor™ monitors) or Perfusion Index (feature specific to Masimo™ monitors) information that is refreshed
on the display. The probe houses an infrared source, red LED and a photo detector. It picks up the signal
from the patient's body. This signal is conditioned by the module to derive the oxygen saturation level and
pulse rate.
(If IPI is enabled and thus no SpO2 waveform is displayed, the SpO2 pulse beat is represented by the green
bar on the left side of the box.)
The pulse oximetry parameters that are shown depend on the probe currently connected to the device.
Before a SpO2 sensor is inserted all parameters for which CAPNOSTREAM™ monitor has been enabled
will show on the Home screen. Once a probe is connected, the device will calibrate its SpO2 function, and
then only the relevant parameters will be displayed. For example, when a probe that supports the standard
SpO2 data is connected to the CAPNOSTREAM™ pulse oximetry port, the CAPNOSTREAM™ monitor will
display only standard SpO2 data. If a probe that measures standard SpO2 data and one other parameter is
connected, the display will show standard SpO2 data and that additional parameter.

3.1.4. Power Supply


The power supply comprises:
• AC-DC converter
• DC-DC converter
• Battery Management circuit
Wall power is supplied to the AC-DC converter, the output of which is fed to the DC-DC converter. The
output of DC-DC converter is fed to all the sections of the monitor for their functioning. The Battery
Management circuit senses the percentage of battery charge and sends this information to the Main board.
This is used to shut down the monitor under low battery conditions.

3.1.5. Display Unit


The monitor has an active matrix color Liquid Crystal Display (LCD) comprising amorphous silicon (Thin
Film Transistor) TFT. It has a built-in backlight that includes long-life lamps. The 6.4" diagonal display area
contains 640 x 480 pixels and can display 262144 colors simultaneously.

3.1.6. Keyboard & Optical Encoder
The monitor has a keyboard with ON/OFF key and four hard keys - Event Admit/ Discharge Pump

Off and Alarm Silence . Soft Menu buttons appear on the screen. The optical encoder on the front
panel is used for the on-screen user interface.

3.1.7. Accessories
CAPNOSTREAM™ monitors supports a line of accessories produced by Oridion Medical for capnography.
FilterLine™ Accessories are henceforth referred to as FilterLine™.
CAPNOSTREAM™ monitors supports different types of sensors for Pulse Oximetry, seen in the photos
below. When choosing sensors, please note that only the sensors appropriate for the pulse oximetry board
in your monitor (Nellcor™/Masimo™) may be used.

3.3. Signal Flow
Switch ON the monitor

AC-DC takes input supply from mains (100/240V) and converts it to +18.3V DC

DC-DC receives +18.3V from AC-DC/ or 14.8V from battery and generates +5V, +12V, -12V, 8.5V & isolated +5V

Chapter 4. Installation
Adhere to the installation requirements for the proper functioning of CAPNOSTREAM™ monitors.

4.2. Installation Site Requirements


4.2.1. Ventilation
To ensure a long operating life, run in a dust free and properly ventilated area. Keep ambient temperature
<35º C and relative humidity in the range of 10 to 95% (non-condensing).

4.2.2. Placement
CAPNOSTREAM™ devices are portable bedside patient monitors. It can be placed on a table, stand, or
trolley or mounted on a pole. Do not place the equipment in an enclosed area that could restrict heat
dissipation from the front or rear of the unit.

4.3. Unpacking
The CAPNOSTREAM™ monitor is properly packed to ensure safety during transport. Read the following
instructions carefully before unpacking.
Carefully remove the CAPNOSTREAM™ monitor and the accessories from the box.
Check that the items on the enclosed packing list are included:

4.4. Product Setup


4.4.1. Installing the Battery Pack
The monitor operates on AC power or on a battery. It is equipped with a rechargeable Lithium–Ion battery
pack. To install the battery pack, open the battery cover on the side of the monitor as shown below.

WARNING: The unit should always be operated with the battery installed in order to provide back-up
power in the event of a momentary or temporary power outage.
Press the release latch and open the battery compartment door.
While holding the battery pack with the wires on the right, rotate the restraining lever up to the horizontal
position and place the battery pack in the monitor.
Push the battery pack all the way in.
Hold the battery pressed in and lock it in position by returning the restraining lever to the vertical position.
Plug the battery cable into the battery connection socket. Push the wires back into the monitor.

Align the flaps on the battery compartment door with the slots in the monitor casing and close the door.
After that, slide the two lockers into position to insure a proper closure of the battery door.
Ensure that the battery pack is fully charged before using the monitor without AC power. A fully charged
battery pack provides 2.5 hours of operation (without printer usage). When the monitor is connected to the
AC mains with the main power switch at the back of the monitor ON (or the monitor plugged in, if it does
not have a back-panel switch), even if the monitor is turned off, the battery pack charges automatically. It
takes approximately 12 hours to fully charge an empty battery pack.
Note: Must charge the battery pack every 3 months for proper operation.
Note2: Before air shipping the CAPNOSTREAM™ monitor with its battery/ battery pack spare part, the
battery should be tested to contain no more than 30% of the full capacity voltages in the battery pack.
A new and unopened Battery Pack spare parts which comes from Oridion already was tested for 30%
capacity and does not require another test.
To test the battery, follow section for testing the battery connections on page 37.
When you start using the monitor, verify that the battery icon at the bottom left of the monitor screen is full.
It is recommended to always have a battery installed. If the battery is not installed, the unit will operate
properly on AC power, but if AC power is lost for any reason the monitor will not work.

4.4.2. Testing the Battery and AC Connections
The battery pack charge level and AC power connections should be confirmed before each use.
Press the ON/OFF ® button to turn on the monitor.
Observe the battery icon level in the bottom left hand corner of the screen.

If you have previously fully charged the battery, the battery icon should indicate that the battery is full.
Note: As part of the monitor power-up, the battery charge level indicator will show full for about 15
seconds after the monitor is turned on. The monitor will then update the battery charge level indicator to
show the true battery level.
Recharge the battery pack when the advisory message BATTERY LOW appears on the display screen. To
recharge the battery, make sure that the monitor is plugged into the AC mains and the power switch on the
rear of the monitor (if your device has one) is turned to the ON position. The orange AC power indicator on
the front panel of the monitor -will light up.
For normal operation, always check that the orange AC power indicator light is on during monitor use. This
will ensure the battery is charged during use and the monitor is prepared in case of a power outage or a
patient transfer. If a patient has to be transferred to another location, the unit can be unplugged and
transferred with the patient. Care should be taken to reconnect the monitor to the AC mains following the
transfer.
• Testing the battery connections before air shipping

Before air shipping the CAPNOSTREAM™ monitor with its battery/ battery pack spare part, the battery
should be tested to contain no more than 30% of the full capacity voltages in the battery pack.
A new and unopened Battery Pack spare parts which comes from Oridion already was tested for 30%
capacity and does not require another test.
To test the voltages, use a DVM as shown below.

4.4.3. Mounting the Monitor


The bottom of the CAPNOSTREAM™ device is designed to fit GCX model FLP-0002-17C Mounting
Adaptor with (100mm) VESA Standard Mounting (can be ordered from Oridion; part number is 010713).
The adaptor fits the GCX model RS-0006-64C Roll Stand Assembly. Please refer to the appropriate
Directions for Use for these products.

WARNING: Do not remove the rubber feet from the bottom of the monitor. The mounting adaptor
includes spacers that allow the monitor to be mounted with the feet in place, ensuring that the air vents on
the bottom of the monitor are not covered, so that the monitor does not overheat.

4.4.4. Turning on the Monitor
This section explains how to turn on the monitor.
WARNING: The monitor is intended only as an adjunct in patient assessment. It must be used in
conjunction with clinical signs and symptoms.
WARNING: Use only Microstream® EtCO2 consumables and approved SpO2 sensors, to ensure that
the monitor functions properly.
WARNING: Do not connect anything other than a SpO2 sensor to the sensor port (for example, do not
attempt to connect a PC to the monitor at the sensor port).
Plug the electrical cord into the mains plug in the rear of the monitor.
Plug the electrical cord into the mains AC supply.
Turn on the power switch on the back of the unit (if your device has one).
The orange power indicator on the front panel will light up.

Press the Power ON/OFF button on the front panel to turn on the monitor. The following happens:
The green power-on indicator lights up, showing that the monitor is turned on.
An hourglass figure appears on the screen for a few seconds followed by the blue monitor salutation
screen for about 5 seconds as the monitor performs a self-test.
The red alarm and yellow alarm lights will briefly light up and the speaker will beep.
If the red and yellow alarm lights do not light up or there is no sound from the speaker, the monitor should
not be used and should be sent for servicing.
You will hear the pump briefly turn on for a few seconds, and then turn itself off. However, if a FilterLine™
is connected to the monitor, the pump will remain on.

4.4.5. Testing CO2 Monitoring


To test the CO2 monitoring function of the monitor, the appropriate FilterLine™ must be connected to the
monitor.
Slide open the FilterLine™ input connector shutter and connect the appropriate FilterLine™.
Connect the FilterLine™ to Breath Simulator as described in the Directions for Use supplied with the
FilterLine™.
When the FilterLine™ is connected, the monitor will immediately begin to search for breaths, but it will not
indicate a No Breath condition before any valid breaths have occurred.

4.4.6. Viewing CO2 Data on the CAPNOSTREAM™


Monitor
The CAPNOSTREAM™ monitor Home screen displays real-time CO2 data. The displayed data includes:
• Real-time EtCO2 and FiCO2 values along with selected unit.

4.4.7. Testing SpO2 Monitoring


To test the SpO2 monitoring function of the monitor, the appropriate sensor must be connected to the
monitor and to the patient.
Connect the SpO2 extension cord firmly to the monitor SpO2 sensor port, and then connect the appropriate
SpO2 sensor to the extension cord.
Connect the SpO2 sensor to the human as described in its Directions for Use.
When the SpO2 sensor is plugged into the extension cable and connected to the monitor, the monitor will
immediately begin to search for a pulse. It will indicate NO PULSE FOUND and SpO2 SENSOR NOT ON
PATIENT until the time that the sensor is placed on the patient. This is classified as a Low Priority Alarm,
and will generate a beep once a minute. To avoid the alarm message and beeping, connect the extension
cable to the monitor, but do not connect the SpO2 sensor to the extension cable until it is time to connect
the patient to the monitor.

4.4.8. Viewing SpO2 Data on the CAPNOSTREAM™


Monitor
The CAPNOSTREAM™ monitor Home screen displays real time SpO2 data. The displayed data includes:
• Pulse Rate
• SpO2 Numeric
• Plethysmograph (SpO2 waveform)

4.4.9. Testing Report Printing


The Print screen is accessed from the Home screen.
The Print screen allows you to choose what report to print, and also to start and stop the printing of
a report.
From the Home screen, select PRINT to display the Print screen.
Use the control knob to select the type of report to print. Only one type of report can be selected at a time.
An asterisk (*) will indicate the report that has been selected. If you choose a case report and no case is
currently active, the field to the right of the report name will read NO CASES.
Choose data to be printed:
Select the PRINT FORMAT option from the Print screen. On the Print Format screen, select the
parameters that you want to print on the report.
Three columns of data appear in one printed report in the tabular formats and two graphs appear in one
printed report in the graphical formats. The print format selected applies to all reports to be printed.
For tabular reports, EtCO2, FiCO2, RR, SpO2, PR, and blank are available for selection. For graphical
reports, EtCO2, RR, SpO2, PR, and blank are available for selection. Selecting blank means that nothing
will be printed in that column.
Click BACK on the Menu bar at the bottom of the screen to return to the Print screen.
Click START PRINTER on the Print screen to begin printing. To stop printing, in order to stop continuous
printing or abort other reports that have not completed, click the STOP PRINTER button on the screen.

4.4.10. Description of The Co2 Board
The monitor uses Oridion MiniMediCO2 or MicroMediCO2 Capnography modules.
The Main board provides 5V to power the module (For the micro 3.3-5v). The module communicates with
Main board using standard non-isolated RS 232-level serial interface. The module provides
inspired/expired carbon dioxide and respiration rate data, which are refreshed on the display every 1
second and CO2 waveform data in real time. The method for measuring CO2 is based on non-dispersive IR
absorption of the CO2 in the breath sample using the Oridion IR source.
The major components of the module are as follows:
• CO2 sensor assembly, including the Oridion long-life IR source.
• Flow System, including pump.
• Microcontroller + peripherals.
• Serial interface.
• Power supply.
• FRS to identify the correct usage of a Microstream™ FilterLine™.
• Input gas connector with the FRS interface.
Before the module begins to measure CO2, it completes the self-test and initialization procedures required
for proper operation. The module enters the power-up mode after powering the module or after external or
software reset. In the Power-up mode, the module performs a set of self-tests. If the module passes the
self-test, it transfers to measurement/initialization mode. If the Main board fails to detect the module during
the self-test, it shuts down. The power-up takes up to 10 seconds, during which time there is no
communication available.
During the self-test, if either of the modules fails to communicate properly or does not pass its self-test, an
error message is displayed; the respective error is displayed as CO2 ERROR on the monitor.

4.4.11. Power Supply Tests

4.5. TFT Display


TFT Display of CAPNOSTREAM™20 monitor is TFT (Thin Film Transistor) active matrix color Liquid
Crystal Display (LCD) comprising amorphous silicon TFT attached to each signal electrode, a driving circuit
and a backlight. It has a built-in backlight that includes long-life lamps. These lamps are replaceable.
The 6.5" diagonal display area contains 640 x 480 pixels and can display 262144 colors simultaneously.
The display features:
• High luminance (300 cd/m2, typical)
• Low reflection
• Wide viewing angle with retardation film (anti-glare treatment)
• Display reverse scan function
• 6-bit digital RGB signals
• Edge type backlight with long-life-lamps (two lamp holders, inverter-less)
• Variable luminance control

4.6. LED Display


The LED Display of CAPNOSTREAM™20P monitor composed of a TFT-LCD panel, driver ICs, control and
power supply circuit board and a backlight unit including LED driving circuit. The 6.5" diagonal display is
designed for industrial applications with VGA (640 X RGB X 480) resolution and 16.2M (RGB 6-bits + FRC)
OR 262k colors (RGB 6-bits).

Chapter 5. DISASSEMBLY GUIDE
This chapter explains the procedure to disassemble the monitor so that faulty assemblies can be removed
and replaced as needed. Do not disassemble the product further beyond the point described in these
procedures. Reassemble the monitor in the reverse order. When assembling the enclosure back onto the
monitor, the CO2 tube should be routed carefully.
Every unit which returns for service needs to pass all the tests in Chapter 7.

5.1. General Disassembly Issues


5.1.1. Removal
Disconnect the Mains supply before disassembling the monitor. Gently remove all the accessories that are
connected to the monitor. Disconnect the internal battery as explained below.
WARNING: Do not pull the wires. Hold firmly on the connector housing.
WARNING: Anti-static precautions are a must for ESD protection.

5.2.1. Battery Replacement

Replace the defective battery with the new battery.
Note: Before air shipping the CAPNOSTREAM™ monitor with its battery/ battery pack spare part, the
battery should be tested to contain no more than 30% of the full capacity voltages in the battery pack.
A new and unopened Battery Pack spare parts which comes from Oridion already was tested for 30%
capacity and does not require another test.
To test the battery, follow section concerning battery testing on page 37.

5.3. Rear Cover


Follow the steps explained in Sections 5.1 General Disassembly issues and 5.2 Battery before
disassembling the rear cover.

5.3.1. Carrying Handle


The Carrying Handle of every unit which comes for repair should pass a visual inspection.
To perform the visual inspection for the Carrying Handle follow the steps explained in Sections 5.1 General
Disassembly issues, 5.2 Battery and 5.3 Rear Cover.

If the Carrying Handle has cracks, breaks or damage replace it.
To assemble the Carrying Handle back on the rear cover, insert the Carrying Handle into its place and
verify that it is seated properly on the rear cover.

Use RTV P/N SP010500 or the RTV which supplied with the Carrying Handle spare part to glue the
Carrying Handle to the rear cover, as shown in Figure 22. RTV for the Carrying Handle, below.

Chapter 6. SERVICE SUPPORT

6.1. Service Support


The monitor provides the technician with on-screen menus to help service the unit. Upgrading the monitor
can also be done using this menu.
Enter the Service menu through SYSTEM SETUP screen by selecting the SERVICE option. To service the
unit:
Select SYSTEM option in the Home Page.
Select SERVICE option in the SYSTEM SETUP screen. The SERVICE ENTRY screen will be seen as
shown in Figure 54. CAPNOSTREAM™ monitor Service Screen, below.

This screen displays information which includes total hours of use for the modules (CO 2 and SpO2), total
number of hours remaining until service and calibration is required (only CO2 module), serial number of
CO2 module, SpO2 module and monitor, and hardware revisions of CO2 module, SpO2 module, and
Monitor.
Other than HOME and ENTER PASSWORD, all the other options are disabled in this screen.
To enter the SERVICE screen, enter the password: SERVICE or SERV.
Entering Password as SERVICE will enable the following options: DEMO MODE, INST DEFAULTS,
SERVICE DATA, SOFTWARE SUPPORT and CO2 CAL.

6.3. Institutional Defaults
WARNING: Setting institutional default alarm limits to extreme high or low values will impair the
alarm system’s effectiveness.
Selecting the INST DEFAULTS option on the SERVICE screen brings up the INST DEFAULTS Screen.
This screen gives 6 soft-menu options: ALARM LIMITS, TREND, MONITOR, CO2, SPO2 and BACK, as
shown in Figure 57. Institutional Defaults Screen, below.
6.3.1.TO UPLOAD OR DOWNLOAD DEFAULTS:

1. For upload, insert a USB Flash Memory drive with at least 2 KB in free memory into the USB
port on CAPNOSTREAM™. For download, insert a USB Flash Memory drive with Institutional
Defaults previously loaded on it into the USB port on CAPNOSTREAM™.
2. Click the SYSTEM menu button to enter the main System screen.
3. Click the SERVICE menu button to enter the Service screen and input the Service password
(see CAPNOSTREAM™ Service Password on page 139).
4. Click the SOFTWARE SUPPORT menu button to enter the Software Support screen. The
Software Support screen can be seen in Figure 47 - Software Support Screen on page 121.
5. Click the UPLOAD DEFAULTS button to upload defaults to a USB Flash Memory drive from
CAPNOSTREAM™ or click the DOWNLOAD DEFAULTS button to download defaults from a
USB Flash Memory drive to CAPNOSTREAM™.
Downloading defaults will create a folder called settings on the USB Flash Memory drive, which will contain
files that can be used to upload defaults to another CAPNOSTREAM™ monitor. After uploading defaults,
the monitor will shut itself off; restart the monitor to resume operation.Note:On selecting RESET, trend will
be cleared.

By selecting the first option “SPO2 MODULE CONNECTED”, two options will appear: “NELLCOR™” or
“MASIMO™”, as shown in Figure 60. Serial Number Screen, below. Select the relevant option.

If the message “SPO2 ERROR” is displayed, and the SpO2 function no longer works, the SpO2
type must be selected as follows
a. From HOME screen, press soft key SYSTEM then soft key SERVICE
b. Enter the password “SERVICE”
c. Press the soft key SERVICE DATA
d. Enter the password “SPO2”
e. Select the appropriate SpO2 type from the dropdown (Masimo™ or Nellcor™)
f. Press BACK until the HOME soft key appears, then press HOME.

6.7. CO2 Direct
Selecting the CO2 DIRECT option on the SOFTWARE SUPPORT screen brings up the CO 2 DIRECT
Screen. The CO2 DIRECT screen is as shown in Figure 62. CO2 Direct Screen, below. Selecting BACK
returns the user to the previous screen.

Once the monitor enters CO2 mode it is the responsibility of the user to monitor its operation. The monitor
will not give any warning or error messages in CO2 mode. The user should SHUTDOWN the monitor once
the CO2 Direct Communication has ended, in order to return the monitor to normal operation.
Note: When CO2 Direct Communication is in progress, the user cannot navigate to any other screen.
After connecting the cable, if the user presses START, Direct Communication with the module is
established and the user can communicate with the module through the RS232 port. The screen displays
the message as shown in Figure 63. Direct Communication Message, below.

Because normal operation is stopped in this mode, the monitor has no way of knowing if a MiniMediCO 2 or
MicroMediCO2 software download was completed properly. Thus it will not exit download mode if a
download aborted, or if corrupted software was downloaded, and it will not be able to respond to normal
commands from the monitor. After a shutdown and restart, if the monitor is unable to communicate with the
CO2 module, it displays a CO2 ERROR message, similar to any other MiniMediCO 2 or MicroMediCO2
modules malfunction.

Selecting the MONITOR SOFTWARE option on the SOFTWARE SUPPORT screen brings up the
MONITOR SOFTWARE Screen. The MONITOR SOFTWARE screen is as shown in Figure 64. Monitor
Software Upgrade Screen, above.
Selecting BACK returns the user to the previous screen.
Note:
The USB Flash Memory drive must be inserted with care in the USB port, without using excessive force. If
the Flash Memory drive cannot be inserted easily into the port, do not use the drive.
Insert a USB device on which the software upgrade was installed in the root folder and press START to
begin software download. Monitor software download will begin as shown in Figure 65. Monitor Software
Download Progress Screen, below.
After pressing START, if no USB device is found, an error message NO USB DEVICE FOUND appears in
the message area and the same screen continues to be displayed.
CAPNOSTREAM™ devices recognize flash memory drive devices manufactured by SanDisk, Lexar,
and PNY Technologies. The monitors will not recognize flash memory drives that have advanced
capabilities which require the installation of additional drivers, such as the SanDisk U3. The capacity
limit for a USB drive that can be used with CAPNOSTREAM™ devices is 8 GB.

When Monitor Software Download is in progress, the user cannot navigate to any other screen. No menu
option appears. Once the software is downloaded successfully, the screen shows a message asking the
user to shut down and restart the monitor so that the new software becomes active.
The user must select the SHUTDOWN option at this time to shut down the monitor or switch OFF the
monitor from the ON/OFF button on the keyboard. When turned on again, the monitor will contain the new
software version.
6.8.1.Upgrade from a version earlier than3.0 to a version 3.0:
Folder UpgradetoV3 has requisites to upgrade a unit to software versions 3.0. UpgradetoV3 contains two
folders, firmware and upgrade.
Copy the two folders on a thumb drive.
Do an upgrade following steps (1) through (6) as in Type-I upgrade. Shut down the unit without removing
the thumb drive and switch it back on. Up to this point screenshots represented in Figure will appear, just
as in the standard upgrade procedure.

On restart, this time the unit will boot an application that will instruct the user to connect the thumb drive
and start firmware upgrade process as shown below in Figure 66. Monitor Software Update Progress

Press START UPDATE to do a firmware download. Shut the unit down after the firmware download has
completed. At this point, both the new boot loader and a version of V3.0 have been uploaded to the unit.
Note: Any software upgrade to a version higher than V3.0 from a version earlier than V3.0 has to undergo
an upgrade to V3.0 as explained in Type-II upgrade; and then an upgrade to a version higher than V3.0 as
explained in Type-I upgrade. Once V3.0 is already installed in the unit one should not try reloading V3.0 by
Type-II upgrade.

6.8.2.Upgrading CAPNOSTREAM™20 & 20P to software rev. 8.5 or higher version


CAPNOSTREAM™20 with software version of 4.5 or above can be upgraded to software version 8.5 or
newer version by following the procedure.
Note: There are features utilizing the hardware of the CAPNOSTREAM™20p, which do not work with the
older CAPNOSTREAM™20 hardware. The major feature in that category which is currently released are
the parameters of Apneas/hr and ODI (ASA). The monitor recognizes the hardware difference and will not
display settings associated with these features. There are features to be released in the future which also
are not available on CAPNOSTREAM™20 hardware.

6.8.3.Software Upgrade Procedure


Ensure that the CAPNOSTREAM™20 monitor has the following software version:
• V5.7A, V5.8, V5.81, V7.8, V8.5.
• 8.6 version has been validated to be upgraded only from this list of versions and no others.
Note: To ensure software compatibility turn the CAPNOSTREAM™20 monitor on and
observe the turn-on screen. If the CAPNOSTREAM™20 monitor has a software version
prior to 4.5, upgrade the software following procedure 6.8.1.
General directions for software upgrade
1. Software version 8.6 is intended for both CAPNOSTREAM™20P and CAPNOSTREAM™20
monitors with the software version mentioned above only.
2. Unzip the folder downloaded from Medtronic™ website. Copy the entire content of the zip file on to
your USB stick.
• Confirm folder named "firmware" is on the USB stick
• The folder must be in the root directory of the USB stick – not in any folder.
• This folder contains two files "checksum.md5" and "image.bin"
• DO NOT change the name of the folder
3. Turn on the CAPNOSTREAM™20P Monitor.
4. Insert the USB Stick with the firmware folder into the CAPNOSTREAM™20 or 20P monitor
5. Navigate to the "SERVICE" screen via the "SYSTEM" screen and enter the password "SERVICE".
6. Select "SOFTWARE SUPPORT -> MONITOR SOFTWARE and select "START"
7. Progress bar will indicate download status
8. After successful download, a message will be displayed to restart the device. Select
"SHUTDOWN"

• Follow standard CAPNOSTREAM™20/20P monitor software download procedure with software


version 8.5 software or newer.

Note: If the message “SPO2 ERROR” is displayed, and the SpO2 function no longer works, follow
Section 6.5, SpO2 Modules for appropriate SpO2 module selection.

Note2: If the CO2 software is lower than 2.50, IPI is not supported. In that case, remove the empty
IPI block from the monitor screens as follows

i. Enter the password “SERVICE”


ii. Press Soft key INST DEFAULTS
iii. Press soft key MONITOR
iv. Set IPI Display to DISABLED using the dropdown menu
v. Press the BACK soft key until at the main service screen and then the HOME soft
key.

Note: The USB Flash Memory drive must be inserted with care in the USB port, without using excessive
force. If the Flash Memory drive cannot be inserted easily into the port, do not use the drive.
Click the UPLOAD DEFAULTS button to upload defaults from a USB Flash Memory drive onto
CAPNOSTREAM™ monitor or click the DOWNLOAD DEFAULTS button to download defaults to a USB
Flash Memory drive from CAPNOSTREAM™ monitor. Downloading defaults will create a folder called
settings on the USB Flash Memory drive, which will contain files that can be used to upload defaults to
another CAPNOSTREAM™ device. After uploading defaults, the monitor will shut itself off; restart the
monitor to resume operation.

Selecting CO2 CAL on the SERVICE screen brings up the CO2 CALIBRATION Screen as shown in Figure

This screen displays the hours before next calibration is due and date & time of last calibration. If either of
these is overdue, it displays the corresponding message. This screen gives three soft menu options: CAL,
CAL CHECK and BACK.

clicked into position.

Chapter 7. PERFORMANCE TESTS
Every unit which returns for service needs to pass all the tests below.
In order to complete the following tests, please fill in the operational test result sheet on page 92 according
the next tests results.

7.1. Visual Inspection


Check that there are no cracks or breaks in the plastic covers of the monitor.

• Set the Manometer to mbar.
• Block the CO2 inlet line as seen in the picture above.
• Connect the digital pump to the exhaust connector.
• Press the Digital Pump, until the Manometer reads 350 mbar (± 20mbar).
• Wait for 1 min for stabilization.
• Start the stopwatch and record manometer reading (V1).
• At the end of one minute, record the manometer reading (V2).
Pass Criteria: (V1 –V2) ≤ 250 mbar

7.4. Buttons and trim knob check
• Press the button and verify that “detailed event” screen is shown.

• Press the button and verify that “ADMIT/DISCHARGE” screen is shown.

• Press the button and verify that “PUMP OFF MODE” screen is shown. To return back to the
home screen press the button again.

• Press the button and verify that the bell on the upper right side of the display is covered with
a red X sign, and the sound is muted.
• Spin the trim knob to the right and to the left and verify that the blue frame moves according to the
trim knob movement.
• Press on the trim knob to verify the enter option is functioning.

7.5. SpO2 functional tests


7.5.1. Nellcor™ units
• Connect the SRC-MAX to the SpO2 connector with an extension cable.
• Use the table below to verify the SpO2 saturation and pulse values which appears on the display
are in tolerance.
• Disconnect the SRC-MAX from the unit and observe that the error message “SpO2 SENSOR
DISCONNECTED” appears.

• When the unit is off, make sure that the battery is in place and the plug is connected.
• Charge the battery by connecting the AC plug, and turning on the AC switch (if applicable).
• After 12 hours of charging, turn off the AC switch (if applicable), and detach the AC plug.
• Turn on the CAPNOSTREAM™ monitor, connect a FilterLine™ and check that the unit still works
after 2.5 hours. If the unit does not work for at least 2.5 hours on battery power, replace the battery.
Note: Must charge the battery pack every 3 months for proper operation.
Note2: Before air shipping the CAPNOSTREAM™ monitor with its battery/ battery pack spare part, the
battery should be tested to contain no more than 30% of the full capacity voltages in the battery pack.
A new and unopened Battery Pack spare parts which comes from Oridion already was tested for 30%
capacity and does not require another test.
To test the battery, follow section 268888339.45.17984184. on page 37.

7.7. CO2 test


The monitor displays the Calibration and Service related information on the Turn-on Screen, Service Home
Screen and Cal Screen.
The screens display the Remaining Hours to Calibration and indicate a Calibration Overdue condition to the
user. The CO2 Calibration screen recommends a date by which Calibration must be done, irrespective of
the hours of usage.
On the Home Screen, a Silent Advisory message CALIBRATION REQUIRED is shown if CO2 calibration is
overdue.
The module is calibrated and the calibration check is done using the CO 2 Cal Menu on the Service Screen.
The Service Screen displays the CO2 Hours Remaining before Service and CO2 Hours Remaining before
Calibration. The monitor takes the user through various steps of calibration. The CO 2 Cal screen gives the
user two options: Calibration or Calibration Check. It also gives the user information on the Date and Time
of Last Calibration and Hours before next calibration is due.
The Calibration sequence includes an Auto Zero, measurement of externally supplied gas, and updating

the internal calibration table. In the calibration mode the module and Main board handshake to optimize the
calibration process. When a calibration check is done, the internal calibration table is not updated, but the
monitor tells the user whether CO2 measured is within specifications or not and whether calibration is
recommended.

7.7.1. CO2 calibration check


Connect a FilterLine™ to the device as shown in Figure 78. FilterLine™ connection, below.
Use the Breath Simulator and ensure that the unit displays the waveform, EtCO2 and RR.
Note: Keep the Breath Simulator OFF during the entire test.

7.8. Calibration Procedure for CO2 Module


Connect the CAPNOSTREAM™ monitor CO2 inlet and CO2 calibration gas cylinder as shown in Figure 80.

Follow the CO2 calibration procedure described below:
• Select SYSYTEM>SERVICE>Enter Password and enter the password “SERV” by rotating the
knob to the relevant letter and pressing on the knob.
• Select CO2 CAL>CAL and Follow the onscreen instructions.
Note: The CO2 calibration is not ready if the CO2 module is still warming up.

The calibration is done when the message “Calibration Successful, Next Calibration Date is XX/XX/XX”
will appear.

7.9. Calibration Interval


The initial calibration needs to be done after 1200 hours of use of the module. Subsequent calibration
should be performed annually or at 4000 operating hours, whichever comes first. Calibration is done when
the Patient Monitor is shipped from the factory.

7.10. Calibration Kit


Calibration should be performed by qualified personnel.
The calibration must be performed with a manufacturer-approved calibration kit.
A manufacturer approved Calibration Kit can be purchased from Air Liquide (Scott Medical). Its part
number is T4653ORF-2BD and it consists of:
• Calibration Gas containing 5% CO2, 21% O2, Bal N2
• Tubing Adapter ("T" Piece)
• Calibration Line (Short Calibration FilterLine™)
If your facility stocks the appropriate gas mix (5% CO2, 21% O2, Balance N2) or you would like to purchase
the gas canister from a local distributor, a calibration tubing kit can be ordered from Oridion. This kit (no.
RS09251) contains all you need for proper calibration of the Microstream™ etCO2 module but does not
include the gas canister.

7.11. Calibration Failures


When any of the following failures occurs, the module will abort calibration.
The error messages that may be displayed on the main screen are:
• Calibration Failed: FilterLine™ not Connected
• Calibration Failed: No gas, wrong gas concentration, or unstable gas
• Calibration Failed: Measurement Error; Check all connections and try again
• Calibration Failed: Calibration aborted by user
• Calibration Failed: CO2 Module Internal Self-Test Failed
The STOP button will be replaced by START after the calibration ends, whether or not it ended
successfully. The BACK button will take the user back to the CO 2 calibration section.

7.12. Printer Check (If Applicable)


• Keep the printer feed button pressed while turning on the CAPNOSTREAM™ monitor until the
printing starts.

• Verify all the letters and symbols are clean and clear.

7.13. Nurse Call Check


• Connect the Nurse Call cable (part number 011157) to the Nurse Call input.
• Connect the two Nurse Call cable probes to the Multimeter.
• Turn on the Multimeter in the diode mode.
• Turn on the CAPNOSTREAM™ monitor and choose the soft button SYSTEM to reach System
Setup.
• Enable the Nurse Call option.
• Return to the home screen.
• Use a CO2 gas canister or breathe into the FilterLine™ so a waveform is shown. Once a waveform
is shown, stop supplying CO2 so a “no breath alarm” will occur.
• Wait 30 seconds until the alarm starts. Once the alarm starts the Multimeter should start beeping.

7.14. D/A Check


• Turn on the Multimeter on the DC voltage measurement mode.
• Turn on the CAPNOSTREAM™ monitor and choose the soft button SYSTEM to reach System
Setup.
• Choose “D/A mode”, and set all the channels to high.
• Check every pin of the Analog connector according to section 3.9.1. Analog Output. For each pin
check for the following results:
• The CAL HIGH voltage readings should be between 0.95v-1.05v (DC).
• The CAL LOW voltage readings should be between 0.00v- 0.08v (DC).
• In the end of the D/A check choose the “D/A CAL RESET” option.

7.15. USB Check


• Turn on the CAPNOSTREAM™ monitor.
• Insert USB flash drive into the USB connector.
Note: The USB Flash Memory drive must be inserted with care in the USB port, without using
excessive force. If the Flash Memory drive cannot be inserted easily into the port, do not use the drive.
• Observe that the symbol is shown on the upper right side of the CAPNOSTREAM™ monitor
screen.

7.16. RS232 Check


• Connect RS232 tester to the unit’s RS232 port, or connect the unit to a PC using the RS232 port.
• Select SYSYTEM>SERVICE>Enter Password and enter the password “SERV” by rotating the
knob to the relevant letter and pressing on the knob.
• Select SOFTWARE SUPPORT>CO2 DIRECT>START.
Note: Use any RS232 tester to verify that the RS232 port communicates or refer to “DATA TRANSFER
PROTOCOLS FOR CAPNOSTREAM™20 AND CAPNOSTREAM™20P” document on the Operator’s
Manual CD (P/N 015124) to set your own communication protocol.

8.6. Preventive Maintenance
Periodic maintenance is recommended according to operating hours:
• The CO2 board in the CAPNOSTREAM™20 monitor should be replaced and sent back to the
manufacturer every 20,000 operating hours and in the CAPNOSTREAM™20p monitor every
30,000 operation hours.
• A CO2 calibration should be performed after the initial 1,200 operating hours of use or one calendar
year (whichever comes first), and following that calibration once a year or every 4,000 operating
hours, whichever comes first.
• The monitor’s remaining number of hours for calibration / maintenance is displayed each time the
unit is powered on. This can also be viewed in the Service Screen.
• Must charge the battery pack every 3 months for proper operation. Although, battery back-up time
of the Li-ion battery may degrade over a period of time. To avoid degradation of battery capacity, it
is recommended that the battery pack be replaced every two years.
Note: Before air shipping the CAPNOSTREAM™ monitor with its battery/ battery pack spare part, the
battery should be tested to contain no more than 30% of the full capacity voltages in the battery pack.
A new and unopened Battery Pack spare parts which comes from Oridion already was tested for 30%
capacity and does not require another test.
To test the battery, follow section on page 37.
