    MEDICAL REPORT IN RESPECT OF:

    RE: OGUNYEMI BLESSING MALE/ADULT

    The above named patient was brought to our hospital on 20/12/2022 after having several
    episodes of convulsions according to history taken. After thorough investigation and laboratory
    tests, he was diagnosed of seizure disorder.

    He is currently on a regular checkups at Beachland Specialist Hospital Arepo ogun state to


    monitor his mental state and undergoing regular therapy. He’s placed on the following
    medications: Carbamazepine, Dilartin, phenobarbital and diazepam.

    In view of his medical status, it would be appreciated, if he can be posted for his PPA in a place
    close to the aforementioned hospital. We believe you will accord him all the necessary
    assistance.
