Dear Engr. Hipe:

Greeting!

This refers to the Notice of Violation issued by your Office to this Facility with Case Number EMB-R1-
NOV-2021-0835-W, regarding our compliance with the Implementing Rules and Regulation (IRR) of
RA 9275 or the “Philippine Clean Water Act of 2004” finding this facility in violation of the
submission of Result of Laboratory Analysis (ROLA).

The facility was able to explain through a letter to your office regarding this matter which is attached
herewith. However, the facility was not able to send a representative to attend a technical
conference, either personally or through online, for the particular reason that most of the staff
during that time tested positive for Covid-19 while others were under home quarantine.

Likewise, we would like also to inform your good office that we have submitted ROLA last 2017.
However, we were not able to submit during 2018, 2019 and 2020 which we were also able to
explain through a letter sent to your office. Furthermore, we were able to attend a technical
conference regarding this matter through Facebook Messenger last October 12, 2020 (Case No.
EMB-R1-NOV-2020-0402-W).

To conclude, in compliance with the Notice, this facility, through its representative have already
attended a Technical Conference last December 09, 2022 at the Provincial Environment and Natural
Resources Office and made commitments to comply with the relevant IRR. Attached herewith is the
accomplished Technical Conference Form.
