Fernando City Assessors Office, DENR-Region1 (Records Section).

4. To process the revision/update and verification of the abovementioned Tax

Declarations.

That I am the co-owner of the above declared tax declarations.

That the required documents by San Fernando City Assessors is required to the Transfer
of Extrajudicial Settlement of Estate dated ___________ of __________________.

That without the following documents our transfer will not go through the revision so
