Lessor/s : Ibarra Ocampo Angon
The Lessor is/are required to submit the following documents for the Alfamart Contract of Lease

Deadline for Submission of Documents:

1. Passport/Valid ID (photocopy)
2. Latest Meralco , Water, PLDT & Other Utility Billings (photocopies)
3. If Title is in Another’s Name, the applicable document giving rights to lease, i.e.:
a. Special Power of Attorney (look for power to lease the specified property, with
power to accept payments)
b. Deed of Sale/Donation or other Instruments of Title
c. Lease Contract
d. Extrajudicial Settlement of estate signed by all the heirs
4. Commercial Zoning Certificate
5. Corporate:
a. Articles of Incorporation (photocopy)
b. Board resolution and/or Secretary's Certificate authorizing lease with Alfamart
6. Site Plan, which may be:
a. Relocation Survey Plan by Geodetic Engineer for Lot Only Properties
b. For Building Lease: as Built Plans
7. For Properties Inside Subdivisions:
a. Proof of full payment of Association Dues
b. Subdivision Deed of Restrictions, with schedule fees, rules and regulations, regulation
of banners and marketing paraphernalia, etc.
c. Homeowner’s Association and/or Neighbor’s Consent
8. Certified True Copy of Title
9. Certified True Copy of Tax Declaration Showing Land for Commercial Use (for Land and
Improvement if building lease, if no improvement, please submit certificate of no improvement);
10. Tax Receipts (photocopy) & Tax Clearance (original)
11. For Building Lease: please submit before Physical Turnover: Lessor’s Building Permit and
Occupancy Permit (photocopies)
12. The following may also be required:
a. Barangay Clearance
b. if mortgaged: Mortgagee’s Consent
c. Laguna Lake Development Authority Certificate when applicable
May be submitted after Contract Signing and before payment of Rent
1. If Sole Proprietorship: DTI Business Name Registration (photocopy)
2. Copy of Mayor’s Permit to conduct business as a Lessor beginning on the year of Alfamart
Opening (photocopy)
3. BIR Certificate of Registration as Lessor, TIN/Vat Registration and Sample O.R. (photocopy)
4. Written notice specifying BDO account for Bank Deposit with photocopy of Passbook
