corporation duly organized and existing under the laws of the Republic

2. During the special meeting of the Board of Directors of the
Corporation held on September 4, 2023, at its office wherein a quorum
was present and acting throughout the following resolution was only
adopted and approved by all members present.

WHEREAS, there is a need to transact, claim and receive with the

SOCIAL SECURITY SYSTEM, PHILHEALTH and PAGIBIG FUND.
“NOW, THEREFORE ME IT RESOLVE AS IT IS HEREBY, to authorize, MS. AIZA AVILA,
to represent the Corporation in the processing of said application and to
sign any and all documents and to execute any and all acts necessary to
effect the foregoing authority.
“RESOLVED FINALLY, to confirm and ratify the acts and things to be
done by the aforementioned personnel pursuant to and in accordance with the
foregoing authorities.
3. The above-stated resolutions are in accordance with the records of the
corporation.
