INTRODUCTION TO PORTFOLIO

The Ateneo de Davao University (ADDU), School of Nursing Clinical Progress Portfolio is
acreative and systematic tool designed for documenting continuous improvement of
student learning experiences. It is a collection of evidence of student’s demonstration of

RLE POLICIES

CLINICAL EXPECTATIONS

The Student Nurses are expected to:

1. Be present and listen to endorsement.


2. Demonstrate a basic knowledge of the physiology involved in the patient's condition
and have a prepared plan of care.
3. Read the patient care plan prior to giving care to the patients and discuss any questions
with the clinical instructor.
4. Take vital signs at appropriate times and document appropriately.
5. Complete the patient care flow sheets at each bedside before leaving each clinical day.
6. Document according to the unit specific policy.
7. Take responsibility for all patient treatments and care as written in the care plan.
8. Be knowledgeable of all the assigned patients’ medications including medication
rationale and side effects.
9. Endorse patient during break time. The clinical instructor should know the status of the
patient(s) prior to all breaks as well as any care that must be completed during the
break
10. Report all pertinent information including changes in the patient’s condition to the
instructor.
11. Provide a comprehensive report about their patient’s to the nurse’s responsible for each
of the patient at the end of the shift.

What did I do today?

Today is my first day of duty in Davao Mental Hospital. I met various patients, some were violent and some were
depressed.. I remember thinking “How do they live life?”I felt it was important to deal with my feelings first – I guess
Ma’am Ana is right to tell me that I need to have self-awareness. I felt quite helpless in the patient’s situation, not
knowing quite what to say and how to respond…

What did I learn?

I learned that life sometimes is not fair. I was able to reflect that I am luckier because I am sane. I have to thank God
that I am given a life that is comfortable. I also am now aware how nurses can best help mentally ill patients.

What do I need to do now?

Perhaps I could research more on how I can become effective student nurse to my patient. There must be some theory
that specifically supports howpsychiatric nurses would best help a mentally ill patient.
