ISSUANCE PROCEDURE:

 For renewals, kindly fill out this application form.

 For newly-issued car sticker:
 Applicable Fees Annual
o 4-wheels (vans, trucks) 250
o 4-wheels (light vehicles & cars) 200
o 3-wheels (tricycle, e-bike) 150
o 2-wheels (motorbike) 100
 A Homeowner to avail the vehicle pass will only qualify if he/she compensate with Association’s membership fee and pay for the monthly
due.
 A Lessee, Caretaker, Renter (Classified as Tenants of San Manuel Villaggio) to avail the vehicle pass will only qualify if he/she pay for the
monthly due.
 For security, stickers will only be issued to San Manuel Villaggio qualified Homeowners and tenants only upon submission of the following
requirements:
o OR and CR of the vehicle/ Driver’s License
o Valid ID
o Proof of Residency
o Endorsement of the awardee or property owner (for tenants)
o Monthly due receipt for current month
 Strict Implementation of “No Sticker Pay Fee Policy” will start on October 1,2022 valid until October 31, 2023.
 The car sticker is non-resalable, non-transferable to other vehicles.
 Only cars and other vehicles in the resident’s name with address at San Manuel Villaggio shall be entitled to use Vehicle Stickers.
However, company vehicle of residents may also be allowed to use Vehicle Stickers provided they are for private use by residents and are
being garaged in their residences
 Stickers installed by temporary means like the use of scotch tape will automatically be confiscated by SMVHOA security.
 All Homeowners and tenants of San Manuel Villaggio who cannot secure their sticker or permits will be charged P10.00 per every entry
inside the subdivision.
 The official sticker design shall be signed by the President of the Homeowners Association Board Officers with a unique control number
visible on the right top corner.
 The official sticker shall be placed only in the presence of an official Officer of the Association for uniformity.
 By signing this form, I hereby acknowledge having read and understood the San Manuel Villaggio Homeowners Association, Inc. data
privacy notice/policy (including how the SMVHOAI will process the personal information and Personal Data I will be providing the
SMVHOAI) and hereby provide consent to the SMVHOAI to process such personal information, in its data privacy notice/policy.
 I hereby declare that I am the registered owner of this/these vehicle(s) / exclusive user of this/these company-issued vehicle.
