I, MANUEL C. SICAT of Legal age, Married, Filipino citizen, a resident of No.

3104 J. B. Juan Street Ugong, Valenzuela City, after having been sworn to in
accordance with law hereby depose and say:

1. I am bonafide member of the Social Security System (SSS) with assigned SSS
number 03-9294925-1;

2. I was an employee of LEIGHTON CONTRACTORS ASIA LTD. with

office address at Brgy. 64 Apostol Street PNR Compound, Caloocan City
from July 4, 2019 to March 18, 2021;

3. I was terminated from employment effective March 18, 2021 due to

Retrenchment or Downsizing;

4. I cannot secure a Notice of Termination from my employer; and.

5. I am executing affidavit to attest to the truth and veracity of the foregoing

statements and to support my application for unemployment insurance or
involuntary separation benefit.
