INSTRUCTIONS:

I. Learning Resources Data Entry


A. Self-Learning Module (SLM) K-12
1. Definition of terms:
a. Self-Learning Module (SLM) - refers to the piece of printed SLMs provided to our learners using the downloaded funds.
b. SLM Printing Status - refers to the status of PRINTED SLMs whether locally managed or central office conformed.
c. Fund Sources - refers to the funds utilized in printing SLM.
b.1. FY 2021 FLO Funds
b.2. FY 2022 FLO Funds (Direct Released & Donwloaded) No. of PRINTED SLM using these fund sources.
b.3. Other DepEd Fund Source from FY2021 & FY2022 (MOOE; GMS; QRF Fund, etc.)
b.4. Other Fund Sources - EXTERNAL (SEF or other funds excluded from listed above)

2. Encoder will follow same instructions in filling-out the template based from the previous Progress Monitoring on Self-Learning Modules (SLMs).
Limit data entry to colored cells and cells with formula, do not remove or replace its content.​
3. Please ensure the validity of data/value that will be encoded in each cell. Otherwise, cells containing formula may change the template and the needed data as well. ​
4. Provide the date of data entry per month.
5. Please avoid unnecessary changes in the given template. Encode only the needed data/value.​

1. Definition of terms:
a. Learning Activity Sheet (LAS) - refers to the piece of printed worksheets/activity sheets provided to our learners as a supplement to other learning resources using the downloaded funds.
b. LAS Printing Status - status of PRINTED LAS.
c. Fund Sources - fund utilized in printing LAS.

b.2. FY 2022 FLO Funds (Direct Released & Donwloaded) No. of PRINTED LAS using these fund sources.
