1. related to processing during start-up of the machine. This is because material being heated in
the machine may decompose and spit in the die. The operator is focused on getting the machine
running satisfactorily and this involves close contact with the machine.
Corrective actions:
 So, great care should be taken at start up. No one should allowed to stand in front of
the die and the hopper lid should be firmly in place, so that the screw cannot be seen or
touched.
 Wear the goggles/face shield.
 No unauthorized person should be in the processing area (new comer not yet trained).
2. Doing maintenance while machine is running. (example, changing the barrel cooling pipe).at
high temperature between 2000c or more there is a big risk that the steam with high pressure it
will land on your face.
Corrective actions:
 Do it when machine is in idle.
 Apply the personal lockout.
 And cordon off the area.
3. Not using the proper type of gloves (anti-cut gloves) or even no gloves at all in opening and
cleaning the pelletizer + no lockout. There is a big risk they will cut their hands or finger.
4. Corrective actions:
 Awareness or trainings to the employees about the lockout tagout procedure and what
types of gloves to be used.
 Regular check in each pelletizer for the anti-cut gloves.
 Supervisor/operators must report if these especial gloves are missing or damage.
