Effective immediately, based on recommendations made by the Respiratory Care Committee


& Medical Director of Respirology, Windsor Regional Hospital has prepared a new patient
referral form for home oxygen, exclusively to accredited companies. These referrals will be
made on a rotating basis and documentation regarding this process will be maintained on the
individual units. Should another accredited home oxygen supplier become available, they
should be directed to make an application to the Respiratory Care Committee / Medical
Director of Respirology for consideration. It is the responsibility of the Program Manager of
the referring unit to monitor utilization.

The main purpose of the above memo is to inform employees (choose 1 only):

a) referrals will be made on a rotating basis


b) the process of patient referrals including the forms used for home oxygen has
changed
c) it is the responsibility of program managers to monitor utilization of oxygen referrals
d) applications for new oxygen suppliers are to be forwarded to the Respiratory Care
Committee for consideration
