AFFIDAVIT OF LOSS

I, JHOEL B. PADILLON, of legal age, single, Filipino and currently

residing at Brgy. Cut-Cut 1st, Capas, Tarlac, after having been duly sworn to in
accordance with law, hereby depose and state: THAT-

1. I am an employee of Accenture Company, assigned at Gateway Cubao.

Relative thereto I was issued a company batch identification card by
Accenture that I usually wear at work attached with a lace;

2. Sometime on August 2023, I discovered that my aforementioned Company

Batch ID issued by the Accenture Company was missing. The same must
have fallen off from my pocket inside the bus when I was travelling from
Capas to Gateway Cubao;

3. I have exerted all efforts to locate my aforesaid Company Batch ID issued

by Accenture Company, but the same cannot be found anymore. Thus, for
all intents and purposes, the aforesaid Company Batch ID issued by
Accenture Company is considered lost;

4. I am executing this affidavit to declare the fact of loss of my aforesaid

Company Batch ID issued by Accenture Company, to secure its
replacement, and for whatever other legal purposes it may serve.
