    A total of 5 agitator vessels (FIGURE Q1) are installed in the chemical plant at Chemical
    Industries Sdn Bhd which manufactures ‘speciality’ chemical that is corrosive in nature.
    Below are the materials used for the construction of the agitator system:
    • Shell, body and support of vessel: SA-240 304
    • Nozzle necks & pipe: SA-312 TP304
    • Flanges: SA-182 F304
    • Fittings: SA-403 WP304
    • Gaskets: Spiral wound with 316 (both outer and inner rings)
    • Shaft: A276-316L

    Based on 3 years record of the reliability and asset management team, all 5 of the agitator
    vessels suffer bending of the agitator shaft and has been classified as a ‘bad actor’.

    With appropriate illustrations, perform root cause analysis (RCA) using 5-whys technique,
    explain your findings in point form and 1 major recommendation. There is a page limit of 1
    page for this assignment submission.
