Greetings!

We write to you for and on behalf of my client, JAHMI UMALI

CUSTODIO with respect to the incident that was happened on September 17,
2022 at around 2:00 o’clock in the afternoon inside your restaurant (TOKYO-
TOKYO) located at Robinson’s Galleria South, San Pedro City, Laguna. Our first
demand letter dated September 28, 2022 was personally sent to you and received
on the same date.

By virtue of a letter dated November 7, 2022 from City Business Permits

and Licensing Office, City of San Pedro, a joint conference was conducted
between you and my client to hear your side and settle the matter amicably.

In connection with said conference, I am formally furnishing you copies of

the doctor’s prescriptions and official receipts of my client’s expenses, as follows:

1. Medical prescription dated October 17, 2022 hereto attached as Annex “A”;
2. Medical prescription dated October 24, 2022 hereto attached as Annex “B”;
3. Receipt dated October 15, 2022 in the amount of Php 2,065.00 duly issued
by ACCUQUICK DIAGNOSTIC CLINIC hereto attached as Annex “C”;
4. Official Receipt with No. 0006215 dated October 17, 2022 in the amount of
Php2,900.00 issued by Crisanto A. Balalta, M.D hereto attached as Annex
“D”;
5. Official Receipt with No. 0006160 dated October 24, 2022 in the amount of
Php2,400.00 duly issued by Crisanto A. Balalta, M.D hereto attached as
Annex “E”;

6. Official Receipt with No. 0006216 dated November 8, 2022 in the amount
of Php500.00 duly issued by Crisanto A. Balalta, M.D hereto attached as
Annex “F”;
7. Receipt dated November 15, 2022 in the amount of Php760.00 duly issued
by ACCUQUICK DIAGNOSTIC CLINIC hereto attached as Annex “G; and
8. Official Receipt with No. 0006217 in the amount of Php500.00 duly issued
by Crisanto A. Balalta, M.D. hereto attached as Annex “H”.

In view thereof, we hereby demand you to pay the sum of Nine Thousand
One Hundred Twenty-Five Pesos (Php9,125.00) for medical consultation,
laboratory services and medicines, together with the expenses my client have
incurred in securing lawyer’s services in the amount of Thirty Thousand Pesos
(Php30,000.00), and Thirty Thousand Pesos (Php30,000.00) as exemplary
damages, or for a total amount of SIXTY-NINE THOUSAND ONE HUNDRED
TWENTY-FIVE PESOS (Php69,125.00) as compensation resulting from the said
incident. We are looking forward to settle this matter the soonest.
