NCM101 Health Assessment


SKILLS LAB

Level Outcome:
At the end of this course, the First year NCM 101 nursing skills students will be able to perform
safe skills that deals with the priciples and technique, given simulated situations in selected
settings, rendering safe and appropriate care utilizing the nursing process.

Learning Outcome:
At the end of this course, the third year NCM 101 nursing skills students will be able to:
1. Systematically and proficiently perform a physical examination of the nose and sinuses
on adults.
2. Correlate physical examination findings with anatomic and physiologic characteristics of
the nose anf sinuses
Objectives:
During the performance of the activity in Assessing the Nose and Sinuses, the nursing students
will be able to:
1. Describe the common methods of physical examination of the skull anf face
2. Gather essential data from examination of the skull and face
3. Perform the procedure with confidence.
General Instructions:
1. Attend the scheduled skills laboratory class wearing a prescribed school uniform
2. Review the skill(s) to be practiced and demonstrated
3. Secure a copy of checklist provided for skills demonstration
4. Gather the equipment neede for skill performance
5. Students will follow safety measures at all times
6. No food or drink is allowed during the online clinical skills class
7. Secure a convenient place where you can perform the procedure to avoid distractions

Return demonstration in assessing the Nose and Sinuses will on Wednesday 7-11am, 3-5pm
during our online/F2F class. Each student will be given a 2 to 5 minutes to perform the
procedure. Video recording during the return demonstration will be automatically saved to
google drive as basis of grading.

Assessing the Nose and Sinuses

1 Assemble equipment and supplies:
 Nasal speculum
 Flashlight/penligth
2 Introduce yourself, and verify the client’s identity.
Explain to the client what you are going to do, why it is necessary,
and how she can cooperate
3 Perform hand hygiene, and observe other appropriate infection
control procedures
4 Identify the patient
Provide for client privacy
5 Determine if the client’s has any history of the following:
 Allergies
 Difficulty breathing through the nose
 Sinus infections
 Injuries to nose or face
 Nosebleeds
 Any medications taken
 Any changes in sense of smell
6 Position the client comfortably- seated, if possible.
ASSESSMENT
Nose
7 Inspect the external nose for any deviations in shape, size, or color
and flaring, or discharge from the nares
8 Lightly palpate the external nose to determine any areas of
tenderness, masses, or displacements of bones and cartilage.
9 Determine patency of both nasal cavities
Ask the client to close the mouth, exert pressure on one naris, and
breath through the opposite naris. Repeat the procedure to assess
patency of the opposite naris
1 Inspect the nasal cavities using a flaslight or a nasal speculum

1 Hold the speculum in your right hand and inspect the client’s left
1 nostril, and in your left hand to inspect the client’s right nostril
1 Tip the clients head back

1 Facing the client. Insert the tip of the closed speculum about 1 cm or
3 up to the point at which the blade widens. Care must be taken to
avoid pressure on the sensitive nasal septum
1 Stabilize the speculum with your index finger against the side of the
4 nose. Use the other hand to position the head and then to hold the
light
1 Open the speculum as much as possible and inspect the floor of the
5 nose, the anterior portion of the septum, the middle meatus, and
midle turbinates. The posterior turbinates rarely is visualized because
of its position
1 Inspect the lining of the nares and the integrity and the position of thr

1 Observe for the presence of redness, swelling, growth, and discharge

1 Palpate the maxillary and frontal sinuses for tenderness
