Skills Laboratory

Level Outcome:
At the end of this course, the second year NCM 107 nursing students will be able to
perform safe basic skills in dealing with normal mother, child, family and population group in any
health care setting with appropriate and holistic care utilizing the nursing process.

Learning Outcome:

At the end of this course, the second year NCM 107 nursing students will be able to:

1. Enhance their knowledge and skills in performing the administration of Crede’s


prophylaxis
2. Apply Crede’s prophylaxis to the newborn’s eye aseptically
3. Demonstrate proper technique during the administration of Crede’s prophylaxis

Objectives:

During the performance of the activity on the administration of Crede’s prophylaxis , the nursing
students will be able to:
1. Understand the purpose of Crede’s prophylaxis
2. Discuss the importance of Crede’s prophylaxis to the newborn’s eyes
3. Prepare materials needed for the Crede’s prophylaxis
4. Apply the eye ointment aseptically to both eyes
5. Practice safe application of eye ointment to newborn’s eyes
6. Perform hand washing before the application of eye ointment
7. Provide sterile gloves during the procedure
8. Perform proper technique in the administration of eye ointment from inner to outer
cantus of the conjunctival sac
9. Observe infection preventive measures during the procedure

General Instructions:

1. Attend the scheduled skills laboratory class with prescribed school uniform
2. Review the skill(s) to be practiced and demonstrated
3. Secure a copy of the checklist provided for skills demonstration
4. Gather the equipment needed for skill performance
5. Students will follow safety measures at all times
6. No food or drink is allowed during the online clinical skill class
7. Review instructions for due dates submissions of assignments and video files
8. May use any materials or improvised materials to make a complete execution of the
procedure
9. Secure a convenient place where you can perform the procedure to avoid any
distractions
10. Students video recorded return demonstration is through the e-learning portal

Specific Instructions:

1. Create a 10 - 20 minutes video on “ Administration of Crede’s prophylaxis ”. Upload


the video in the e-learning portal/LMS Moodle or you may save the video in your official
uphsl email account.

2. Provide and attached the link in the reflections. Reflections should be submitted in the
moodle on or before Oct. 31, 2021, @ 5 pm. Indicate the filename following this format:
 Surname_First Name_Skills_Crede’s Prophylaxis

3. Follow the checklist attached below.

CHECKLIST FOR ADMINISTRATION OF CREDE’S PROPHYLAXIS

PROCEDURE SCORE REMARKS

1. Assess:
 Clinical signs of eye infection
 Assess mother the Obstetrical history
 Clinical signs of hypothermia
 Site most appropriate for the procedure
 Factors that may alter core body temperature
2. Prepare the necessary materials needed:
 Sterile gloves
 Opthalmic ointment or drops
 Sterile cotton or sterile gauze
 Soap & water
 Kidney basin or waste receptacle
Verify the type of prophylaxis to be used.
3. Gather the supplies. Check for the expiration date
4. Ensure that the medication is clearly labeled
5. Observe aseptic technique in the entire procedure.
6. Perform hand hygiene
7. Put on sterile gloves
8. Place the infant in a supine position.
9. Clean the eyes.
10. Stabilize the head with one hand and pull down the conjunctival
sac.
11. With one hand, instill the prophylaxis. One drop at a time.
12. Do the same procedure on the other eye.
13. Do not instill the solution directly on the cornea.
14. Be careful not to drop any of the medication on the infant’s cheek.
15. If eye ointment will be used, open the eyelids.
16. Apply ointment from the inner to outer canthus.
17. Make sure that the tip of the ointment tube will not touch the eyelids
of the infant.
18. Avoid the infant’s hands to get contact with his eyes.
19. Make patient comfortable and safe.
20. Remove gloves and perform handwashing.
Document the procedure.
