LEARNING CONTRACT for

Theoretical Foundations in Nursing
For the entire 1st semester academic year 2023-2024, I abide by the following:

1. I have read and understood all learning outcomes of the course as stated in the course syllabus
2. I agree that the content of the synchronous and asynchronous discussions are synonymous with the modules of the course
3. I am responsible to study in ADVANCE all modules prior to synchronous sessions
4. I am to answer all assigned tasks/activities within the assigned weeks
5. I am to take all major exams FACE-to-FACE
6. If I fail to take a major exam, I have 1 week to request in writing for a special exam – A Special Exam is privileged and NOT
A RIGHT!
7. I am clearly aware that all synchronous sessions will be conducted with an OPEN VIDEO and I must actively participate in all
of them
8. I consent to have my video taken and recorded for educational purposes
9. I am aware that asynchronous sessions are for required independent study hours and must be used for the completion of all
assigned tasks/projects of this subject for a particular period
10. I am aware that if I cannot attend any sessions, I am to send an email to my professor before the scheduled online synchronous
session;
11. I will follow the scheduled session according to the MODULES in the Canvas LMS and that announced by our class
professors.
12. I will be able to see my scores in major exams and will be asked to sign as proof that I am aware of my grades in a specific
period in the course.
13. I am aware of the grading system as stated in the Student Handbook

Most especially I will strongly respect each other’s time by communicating within office hours (8am-5pm) and maintaining
quiet restful weekends for all of us to recuperate from all academic activities.
