RECOMMENDATIONS

1. A pre-trial plan to be drawn up, identifying all systems to be tested and the sequence
of testing. This is to be made available to all relevant personnel.

2. No equipment is to be tested, adjusted or otherwise interfered with without the


express permission of the Master and Chief Engineer. This is to be made clear to all
relevant personnel.

3. Tugs to be immediately available and for the exclusive use of the “Dubai” during
basin trials and to assist the vessel in and out of port.

4. All major items of machinery should be proved fully functional prior to departure
from the berth.

5. Emergency stops and shut-offs to be proved fully functional prior to departure from
the berth.

6. Adequate fire fighting and detection equipment to be in place and proved fully
functional prior to departure from berth.

7. Anchors to be proved fully functional prior to departure from berth.

8. Steering to be proved fully functional prior to departure from berth.

9. All navigation equipment, including radars, AIS, echo sounder, communications


equipment, position finding equipment etc. to be proved fully functional prior to
departure from berth.

10. All machinery monitoring equipment to be proved fully functional prior to departure
from berth.

11. A hazard identification and risk assessment be undertaken prior to trials commencing.
Particular emphasis to be placed on the restricted confines of the port and
contingencies for equipment malfunction.

12. Port Authority to be informed of position and status at regular intervals and requested
to advise Pilot and Master of all expected traffic movements within the area.

13. Trials not to take place in the hours of darkness or inclement weather

14. On no occasion should propulsion equipment be operated in a manner that could


cause grounding, collision or contact in the event of equipment malfunction.

15. Vessel to be manned with a full compliment of qualified Crew.

16. Navigation lights to be displayed to conform with the latest “International Regulations
for the Prevention of Collisions at Sea”.

17. Stability calculations to be carried out prior to the vessel’s departure from Jebel Ali.
18. All procedures and operational limiting criteria as laid out in the procedure are to be
fully complied with by all personnel involved with the operation.

19. If it is proposed, or considered necessary to deviate from the reviewed procedures, the
Salvage Association is to be advised and further approval sought.
