I, Muthu Kumar S/O Ravi, Residing at No. 6 B-Block MM colony Aminjikarai

Chennai 600026, do hereby solemnly affirm and sincerely declare as follows:

I, hereby state that when I was driving my two-wheeler bearing the REG NO: TN
02 AT 6608 near Vanagaram on April 4, 2023, around 11.59 am, another AURA
car bearing the vehicle REG NO: TN 10 BK 7882, which is under the care of FPL
AUTOMOBILES PVT LTD.
At the time of the accident, I (DL NO-TN02 2011 0005997) drove and came from
the opposite side, lost control and hit the AURA vehicle (TN 10 BK 7882), which
caused damages on the right side portion of the car. After this collision, I felt down
and got injured on my leg. This incident occurred due to my inattentiveness, so the
owner of the TN 10 BK 7882 or FPL AUTOMOBILES PVT LTD will not take
any responsibility for what happened because of me.

Henceforth, I will not take any further action which took placed, I will take full
responsibility for my expenses, and I will accept the contribution of Rs.4, 000
provided by FPL AUTOMOBILES PVT LTD for my act of compromise towards
this event.
