EXTENDED MAINTENANCE
AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This AGREEMENT is entered into and executed this ___________________, by and between:

FMH ANIMAL CLINIC, LAS PINAS, a legal entity organized and existing under and by virtue of the laws of the
Philippines with office and postal address at Las Pinas, herein represented by its Owner, DR. ATO HERNANDEZ, hereafter referred to
as “FIRST PARTY”,

- AND -

MAN MEDICAL PHARMA AND DIAGNOSTICS, INC. (MAN PHARMA), a corporation organized and existing under
and by virtue of the laws of the Philippines, with business address at No. 200 J. Elizalde St., Phase V, BF Homes, Parañaque City, herein
represented by its Chief Operations Officer, MR. ALBERT ANTHONY G. CATRAL, hereinafter referred to as the “MAN
PHARMA”.
WITNESSETH:

WHEREAS, FIRST PARTY is the owner of the machine and its accessories particularly described hereunder:

Total EMA Price: Php 63,000.00

NOW, THEREFORE, FIRST PARTY and the MAN PHARMA hereby enter into this Agreement under the following
terms and conditions:

1. The parties agree that this Extended Maintenance Agreement shall cover the period from SEPTEMBER 15, 2023
to SEPTEMBER 14, 2025 or TWO (2) Years upon the expiration of the TWO (2) year warranty period;

2. The FIRST PARTY hereby purchased from MAN PHARMA the Extended Maintenance Agreement in the total
amount of SIXTY-THREE THOUSAND PESOS (PHP 63,000.00) VAT INCLUSIVE pursuant to this Extended
Maintenance Agreement;

3. The FIRST PARTY is guaranteed under this Extended Maintenance Agreement Package the following benefits /
services:

a. Instrument repair or replacement

b. No shipping charges for replacement equipment
c. IDEXX Technical Support
d. JARCET Engineer Customer Support
e. Regular Preventive Maintenance
f. Dedicated Sales Support
g. Regular Software Updates
h. Instrument training in-house and online
i. Full staff trainings at your practice
