Re: Mrs. Sylvia Meadows

Thank you for seeing Mrs. Meadows, a 48-year-old clerical worker who
is being discharged from the emergency department after overnight
observation. At that time, the diagnosis of pulmonary embolism was
excluded. I would be grateful if you could notify Mrs. Meadows that she
may have an endocrine disorder related to the attacks of Crohn’s
disease or an autoimmune disease that need an endocrinological
review.

Yesterday, Mrs. Meadows was admitted to the emergency department


a month history of shortness of breath triggered by vomiting attacks
but did not relate to exertion. In addition, she was concerned about
dizziness and trembling. Her sodium level was low, while her potassium
level was high. Chest X-ray showed no abnormality, and glycosylated
hemoglobin was in the normal range.
Today, Mrs. Meadows was discharged after she was notified that she is
going to be updated with the endocrinologist’s appointment details.
Could you explain to Mrs. Meadows her condition and the reason for
referring her to an endocrinologist.
