CONTRACT OF BLESSED JESUS VILLAVERDE ANDRES ACADEMIC SCHOLARSHIP

SEMESTER S.Y.

I, REANNE DOMINIQUE GUINTO, 20 years old, Filipino, with address at

144/659 SANTERO SUBDIVISION, HABAY 1, BACOOR CITY, CAVITE, having been admitted as an academic
scholar in the BS in ACCOUNTANCY of Colegio de San Juan de Letran - Manila do hereby voluntarily agree and
accept the following terms and conditions:

1. That I shall adhere to the policies of the BS in ACCOUNTANCY as well as the policies indicated in the
Collegiate Student Handbook.
2. That I shall comply with all the academic requirements prescribed in this scholarship contract.
3. That non-compliance in any of the academic requirements as required by the program would mean revocation
of my scholarship.
4. That in case of my voluntary renunciation of the scholarship program, I shall reimburse all the expenses
incurred by the Colegio during my stay in the program.
5. That this contract will be for the duration of the whole program of BS in ACCOUNTANCY
6. 6. Retention Policy
To be able to maintain the scholarship, the student must:

• Have a semestral average of 90% with no grade lower than 88% in all courses
• Have not obtained Authorized Withdrawal (AW) or Unauthorized Withdrawal (UW) in any course
• Carry the full load prescribed by the program in a given semester
• Have not committed any major offense prescribed in the Collegiate Student Handbook 7. Other
Requirements:
• Regular class Attendance
• Active membership or officership in a Recognized Student Organization (RSO)
• Regular attendance in Academic Scholars Activities

That I after the graduation from the BS in ACCOUNTANCY Program and having found a stable job within three (3)
years, as a sign of my gratitude and appreciation to the Colegio, I am willing to help in the scholarship program
being offered by the Colegio de San Juan de Letran – Manila.
