The survey results provide a health check for the Forum. Many Forums use the results to focus on areas of
improvement.

Notes: Many Forums perform this survey annually and save the results in order to have an ongoing
record of the Forum’s health and life cycle.

Facilitator Instructions
1) Give each member a copy of the survey. Tell them the survey is meant to be anonymous so they should NOT
write their name on it!
2) Ask each person to circle the number that most closely represents their impressions of the Forum.
3) After about 10 minutes, collect the forms and tabulate the results.
4) Share the results with the Forum members (write on Flip chart).
5) Lead the Forum in a discussion of strengths and opportunities for improvement. Ideally, select three areas
where the Forum would like to improve over the next year.

Notes: Ideally, this survey should be administered and the results should be compiled before the
retreat. At the retreat, the results can be discussed as a developmental exercise for the
Forum.

2) Collect the results and compile them before the Forum retreat.
3) At the retreat, lead the Forum in a discussion of the survey results. Focus on strengths and opportunities for
improvement. Ideally, select three areas where the Forum would like to improve over the next year.
