Goals of therapy
B/I Identify cause
Control HR/BP/volume status
Asymptomatic Guideline mandated medications
Decrease hospitalizations
• ACEI Decrease mortality
• SGLT-2 inhibitor Increase quality of life
Medications if indicated
Statin if ASCVD present or risk ≥10%
C/II-III Aspirin if clinical ASCVD present
Symptomatic Anticoagulation if LV clot, a-fib
or previous embolic event
• ACEI Interventions if indicated
• SGLT-2 inhibitor Surgery: revascularization or valve surgery
• Beta blocker ICD: Class I-III or C with an EF≤30% and have life
• Furosemide expectancy >6 months, ↓sudden death, ↓mortality
• *Spironolactone CRT: ≤35%, QRS>150, symptoms despite max medical
therapy. ↑QOL, ↓mortality, ↓hospitalizations
