We the undersigned after having been duly sworn to testify, as follows:

Q. What are your names and other personal circumstances?

A. We are PM.Sgt Eugene Demetrio and PCpl Joshua Gaitano both of legal age, single
and presently assigned with the Butuan City Police Station 2, BCPO-2

Q. Do you know PLTCOL Newid Coloyan, the applicant for search warrant?

A. Yes he is the Chief of Police of Butuan City Police Station 2

Q. Do you know the Premises of the suspect Bonn Lloyd Gallares?

A. Yes he lives at Purok 3-C Ampayon, Butuan City

Q. Do you have personal knowledge that in the said premises the following properties
are being kept, being used or intend to be used without proper document, to wit: Pipe
Bomb, Mk 2 grenade ?

A. Yes

Q. Do you know who is or who is the persons who have control of him above describe
properties?

A. Yes he is Bonn Lloyd Gallares AKA “Kokey” 43 years old, single, retired Police
Officer and a Resident of Purok 3c Ampayon, Butuan City

Q. how do you know that the said properties are kept in his premises which are the
subject of the offense

A. We conducted discreet surveillance and it was confirmed that Juleo A Cane is

keeping Pipe Bomb, Mk 2 grenade in his residence
