NOTICE TO EXPLAIN

It has come to our attention, through a complaint, that you have violated the foregoing
provision of our company’s table of offenses:

E. Offenses that Affect the Health, Safety and Security of the Company’s Employees

8. Violation of safety rules and regulations or causing another person to violate a

safety rule regulation whether or not it endangers the life of other people or to one’s
self or it endangers company property

Summary of Incident:

Earlier today at around 10:30 AM, one of your construction worker slipped from the
temporary ladder while going down from the second floor. The subject was tasked to
undertake curing of the slab by watering it. Height of the fall was approximately 3 meters
and the impact with the ground was absorbed by the side of the body based on eye
witnesses.

Based on initial investigation the possible cause of his slipped from the temporary ladder
as the subject was not wearing protective gear during his duty.

Given the above-stated narration, you are hereby enjoined to give us an explanation within
48 hours as to why suitable action should not be taken against you for the act stated above.
Failure on your part to submit your written explanation within the period specified above
shall be construed as a waiver on your right to be heard which shall render the
Management to decide on your case based on the evidence the Management has at hand.

The Management does not tolerate acts that are detrimental to the Company and its
employees and should we find that you are guilty of the above-stated violation, you shall be
dealt with the corresponding penalty.
