- Educate the patient/ client what exactly their health condition is, what the risk factors are, and how to prevent other issues from arising. - Teach the patients and their families how to properly administer medications. - Guide them about the appropriate dietary and lifestyle modifications they need to adhere. - Inform them of the hazards associated with their illness especially for immunocompromised patients. - Educate the clients about the technologies of self-monitoring devices they can use to keep track of their health status.
To reduce the morbidity and mortality rate, it is important to:
- Assess the living conditions on specific communities to better understand and plan what the needs of their people are. - Educate the clients of the preventive measures they can do at home, in the workplace or in their community to avoid the spread of diseases. - Teach them of ways to ensure the safety and improve the health of patients who already have medical conditions. - Edify the clients about the importance of good personal hygiene and sanitary standards to prevent the spread of illness and disease. - Guide them in the correct ways to improve sexual and reproductive health and encourage healthy lifestyle and activities.
