participation in The City School’s trip to ……………………..from ………………

I understand that this trip is designed to provide an opportunity to students to learn practical and useful
information that will broaden their perspective and expose them to new ideas.

I am aware that The City School will make all normal and necessary efforts to ensure the safety and security
of the students during the trip as may be required and that The City School shall in no event be liable for any
direct, indirect, incidental, special or consequential damages, losses and mishaps arising out of or relating to
this trip.

My child has no illness, allergies or disability that may require special attention, other than those already
notified to the school.

I agree to reimburse the following:

 Any emergency medical expense incurred by The City School for my child.
 Any expense incurred by The City School because of my child’s misbehaviour during the trip.

I absolve the school of all responsibilities beyond that of normal supervision and duty of care.

I hereby release The City School , its directors, officers, employees, agents and members ,from any liabilities,
actions, proceedings, losses, costs, damages, expenses, claims and demands arising out of or relating to this
trip.
