Bicolandia Designing Resilience Health Care Facilities: A Specialized Pneumology And


Tertiary Hospital Through Humanistic Architecture

Introduction

All of our citizens are entitled to the fundamental right to good health under our constitution.
While healthcare is dependent on the ability of each citizen to pay. Inequity in health is a very
real problem in our society. In our nation, the standard of healthcare for the wealthy differs
greatly from that for the poor, and this gap is widening. Every nation, developed or developing,
places a high priority on public health. The improvement of health conditions in developing
countries is also given top importance by the international community, both public and private.

The economic and social well-being of society is greatly influenced by hospitals .They are
equipped to restore a person's productivity thanks to their mending and restorative processes in
the community. The hospital's ability to successfully carry out its healing goal is crucial. The
characteristics of the physical environment in which a patient receives care have an impact on
patient outcomes, patient satisfaction, patient safety, staff efficiency, and organizational
outcomes. Hospitals are designed to support and aid patient safety, advanced medicine and
technology, and quality patient care, as well as to envelop the patient, family, and caregivers in a
psycho-socially supportive therapeutic environment. Both positive and negative consequences
are possible. Environments are never neutral.

Health services research is a multidisciplinary field of scientific inquiry that investigates how
social factors, financing systems, organizational structures and processes, health technologies,
and personal behaviors influence access to health care, health care quality and cost, and,
ultimately, our health and well-being. Individuals, families, organizations, institutions,
communities, and populations are its research domains.

Background of the Study

The healthcare system in Bicol needs improvement, and building a new hospital in Legaspi is a
step towards achieving that goal. This new hospital will benefit not only the people of Albay but
also the neighboring provinces in Bicol.( QUISMORIO 2023)

LEGAZPI CITY -- Tuberculosis (TB) remains to be a health problem that needs to be addressed in
Bicol, as close to 11,000 people were reported to be afflicted with this lung disease last year, the
Department of Health (DOH) in the region said DOH data indicated that 10,614 TB cases were
recorded in 2018 and of this figure, 278 died, according to Dr. Ernie Vera, DOH regional director .He
said the agency is still collating the data reported from various provincial health units in Bicol for the
first semester of this year Out of 10,514 cases of tuberculosis recorded in Bicol, Camarines Sur has the
highest with 3,685; followed by Albay, 2,756 cases; Masbate, 1,537; Sorsogon, 1,278; Camarines
Norte, 959,
And Catanduanes 399.

According to the 2018 World Health Organization (WHO) report, TB is one of the leading causes of
death worldwide and it is the leading cause of death from a single infectious agent.Vera, however, said:
“As of June 2019, the treatment success rate for TB in the whole region is 93 percent, and we are
hoping and aiming to have a drastic drop in cases of TB and fully prevent death caused by the disease,
TB is a curable disease and through combined efforts, we believe that we will be able to accelerate,
and sustain TB response,” he added.

In Bicol, among the leading killer diseases are heart ailments, pneumonia, hypertension, tumors,
tuberculosis, trauma/injuries, kidney disease, cerebrovascular, diabetes and septicemia. The WHO
report said a total of 1 million Filipinos or 1% of the national population have active TB and many of
them do not even know that they are carriers of the disease. Around 70 Filipinos die daily or 27,000
Filipinos annually from TB, which is supposed to be a highly-curable disease.

With around 113.2 thousand cases in the Bicol area of the Philippines in 2020, acute respiratory
infection was the most common ailment there. Hypertension and fever of undetermined origin were the
other main causes of morbidity in the area.

Although it is widely believed that medical care significantly reduces mortality, the impacts of medical
services on health are frequently underappreciated since some of the same characteristics that drive
demand for primary health care (PHC) services are also linked to higher rates of morbidity and death.
Determining the factors that influence the demand for medical services is crucial for assessing health
outcomes.

The strategies chosen to provide health services to rural areas in low-income countries are motivated in
part by the following widely held beliefs about the health conditions and patterns of demand for
medical services in such countries: Many Third World health problems are thought to be susceptible to
elementary types of care and simple drugs that can be supplied by paraprofessionals; Conventional
modern medical resources are thought to be located primarily in urban areas; Many Third World health
problems are thought to be susceptible to basic types of care and simple the notion that straightforward
solutions, typically preventive in nature, can be used to address many issues in rural health
Statement of the Problem

Increasing access to suitable and adequate health facilities for the underprivileged and
marginalized parts of society is a significant concern for the Philippine health system. However,
with the institutionalization of the Department of Health Facility Enhancement Program (DOH-
FEP), everyone will have access to high-quality, low-cost healthcare. Since health facilities have
been so blatantly neglected and poorly managed in Bicol .

Lack of resources, health facilities and full ignorance of standards are the most frequent
problems.

The study opted to answer the following minor and major problems that are chronologically
stated as follows:

1. What factors must be considered for the implementation of the project, and the
various zones?
2. Providing them with access to medical care services, whenever and wherever they
need it, even in times of disasters.

LITERATURE REVIEW

The architecture of health unites buildings for medical, rehabilitation, and health institutions, in which
a special architectural environment is created that contributes to a full-fledged healing process with all
of its components (modern medical technologies and engineering systems, the setting and arrangement
of premises and services, interiors).

Health architecture is the art of untangling many technical flows in order to create a high- tech,
comfortable, and visually appealing environment for both patients and employees.

A contemporary notion that has gained popularity among hospitals is to vigorously promote wellness
rather than merely treating disorders. The infrastructure design of the hospital is very crucial for a
better outcome of patient results, which includes patient safety. The intended hospital structure is a
once-in-a-lifetime design, and substantial alterations cannot be implemented later, therefore the
thought process must be completed at the outset, taking into account all of the various diverse
elements. The building's infrastructure should be appealing, functional, and respond to the needs of
patient care.

RIGHT COMPONENTS ON FACILITY DESIGN, Despite the fact that millions of dollars were spent
on the development of these facilities, little consideration was given to facility design, which harmed
the functional systems within the hospital as well as patient safety. The proper components considered
for the facility's design and planning have a very significant and beneficial impact, which increases
human performance and, in particular, the safety of employees, patients, and their families.

PLANNING OF THE FACILITY IN ADVANCE, To create a better environment in the facility,


the team members, namely the medical planner, engineers, architect, interior designer, site and
landscape designer, should take overall responsibility, as they are responsible for the overall
design that will affect the patients, families, and employees in a positive way throughout the
facility.

Resilient design is the practice of designing buildings, landscapes, and entire communities to
reduce the impact of extreme weather and other external hazards. Resilient design prioritizes
practical and realistic solutions.

Resilience is a technique for improving a building's, facility's, or community's ability to both


avoid and recover from damage. Each structure is intended to serve a specific purpose. Each
function, each tenant, has a set of requirements and support systems that are required for that
function. In developing the specialized resilience strategy and design, consideration should be
given to those specific needs. Achieving resilience for tenants will require a combination of
physical building attributes that determine the structure's ability to resist disruptive events and
programs and procedures that minimize impact and

enable a timely return to operations. Both sorts of resilience accomplishment aspects should be
anticipated and incorporated into an effective building design.

Since we are positioned on the eastern side of the Pacific, typhoons, whether small or strong,
travel past us (Bicol Region). We have built a culture of constantly being prepared for typhoons
throughout the years, which is why developing a resilient health care facility in Bicol is crucial.

Research methodology

The range of architectural options and technologies used in today's resiliency constructions is
outstanding. Due to the difficulty of the task, conducting an in-depth analysis of the technological
innovations utilized in these structures calls for a special technique. Thus, the following components
of a research methodology were created:

 Gathering information on the most recent technologies utilized in resilient structures today
and compiling images and documentation;
 • An analysis of the collected data based on the literature, addressing the problem of advanced
technology used in finished constructions generally.
 • Using technology that was significantly more advanced than that employed in earlier
projects, conducting architectural studies of selected Tertiarty Hospital buildings in line with
the building code.

Site

 The name Bicol first came to be known as the name of the biggest and longest river in
the region. The river comes from the outflow of lakes and springs from the provinces of
Albay, Camarines Sur and Camarines Norte and form a large stream that exits in the San
Miguel Bay, off the shores of Camarines Sur. This paper estimates the parameters of a
simple model of the demand for health services using data from the Bicol Multipurpose
Survey data from the Philippines.
Naga City is famously referred to as the Heart of Bicol for being the regional center of
commerce and trade, culture, and religion.

 Valenzuela officially the City of Valenzuela (Filipino: Lungsod ng Valenzuela), is a 1st
class highly urbanized city in the National Capital Region.Paso De Blas is a barangay of
Valenzuela City in the Third District within the National Capital Region (NCR) of the
Philippines.
 Panglao , Bohol , Cebu
Philippines
 Panglao, officially the Municipality of Panglao is a 4th class municipality in the province
of Bohol, Philippines.

Site Selection
Site Criteria
Listed below are the general and specific site characteristics that aided the
researcher in selecting the most suitable location for the proposed project. Choosing
a site is not easy; it should meet the requirement of the desired program.
1. Accessibility and Proximity – It is defined as easily reached or entered
physically, refers to the public transportation and refers to the closeness to users.
2. Availability – This term deals with presence of important utilities such as water
supply, power, and community lines, telephones, and drainage.
3. Flexibility – It is described with sufficient space and can adapt changes for
future expansion.
4. Environment – It is the key aspect in rehabilitating the people.
5. Geographical Location – It is the point of view in the area.
6. Topography – land character of the site.
7. Zoning – It should be permitted to be used as retreat facility
8. Views - Site design should maximize views of natural features and minimize views
of visitor and support facilities.
9. Natural Hazards - Development should be located with consideration of natural
hazards such as precipitous slopes, dangerous animals and plants, and hazardous
water areas.
Specific Criteria:
1. Site should be accessible to resident, visitors and guests
2. Site should be accessible along major or minor line of circulation
3. Site must have effective waterline, power supply, communication, facilities
and other essential utilities
4. Size, including ability to accommodate future expansion
5. Environmental issues and remediation requirements, if any
6. Relationship to surrounding proposed future land uses
7. Relationship to municipal planning initiatives
8. Real estate catalyst potential for the surroundings areas
9. Vehicular traffic patterns (both public and service)
10. Parking
11. Pedestrian circulation patterns/ease of pedestrian access
12. Utilities
13. Sustainability considerations
